Twin Pregnancy: Early Signs, Symptoms and What to Expect

A few clues can raise suspicion of twins before a scan, but none of them confirm it. Higher levels of hCG (human chorionic gonadotropin) and alpha-fetoprotein on bloodwork are sometimes seen in twin pregnancies, and a provider hearing what sounds like more than one heartbeat on a Doppler is another possible tip-off. Rapid early weight gain and a uterus that's measuring ahead of schedule can also nudge a provider toward ordering an earlier ultrasound.

These signs of twins are reasons your provider might look closer, not proof by themselves. Hormone levels and fundal height vary a lot between individual pregnancies, so the only way to know for certain is an ultrasound.

A twin pregnancy is confirmed by ultrasound, not by symptoms. Most people first learn they're expecting twins at the dating scan, typically done around 8 weeks to confirm gestational age and count how many gestational sacs and heartbeats are present.

The next major twin pregnancy scan, the chorionicity scan, usually happens between 10 and 14 weeks. This scan looks at the number of placentas and the shape of the membrane where it meets the placenta (a thick "lambda" sign points to twins with separate placentas, while a thin "T-sign" points to twins sharing one). Getting this scan done early matters, because chorionicity becomes much harder to determine accurately as pregnancy progresses.

Twin pregnancy care generally means more frequent appointments, added nutritional support, and closer watching for the complications that are more common with two babies rather than one: preeclampsia, gestational diabetes, and especially preterm labor.

Nutrition and Weight Gain With Twins

Carrying twins raises caloric needs by roughly 40%, but that's based on your pre-pregnancy body size and health, not simply "eating for three." Extra iron and folic acid are commonly recommended beyond a standard prenatal vitamin, since the risk of anemia is higher with twins, along with an emphasis on protein-rich foods to support two babies' growth.

Your provider or a dietitian will set individualized calorie and supplement targets based on your starting weight and bloodwork. There's no single "twin diet" that applies to everyone the same way.

Extra Monitoring for Preterm Labor and Complications

Twin pregnancies carry a meaningfully higher chance of preterm birth; over half of twin pregnancies deliver before 37 weeks, compared to a much smaller share of singleton pregnancies. Because of this, providers often check cervical length more regularly and watch closely for early signs of labor, along with screening more often for blood pressure changes linked to preeclampsia.

Between scheduled visits, a few signs are worth calling about rather than waiting: regular contractions before 37 weeks, vaginal bleeding, a sudden gush or trickle of fluid, a severe or persistent headache, sudden swelling in the face or hands, or a noticeable drop in either baby's movement. These don't automatically mean something is wrong, but with twin pregnancies carrying higher complication rates, it's worth getting checked rather than waiting it out.

Twin pregnancies come with a scan schedule that's noticeably busier than a singleton pregnancy, and keeping track of which scan happens when, especially the time-sensitive chorionicity window, can feel like its own logistical challenge on top of everything else.

Diagnostics: 8,500+ NABL-Accredited Labs and Imaging Centres

Expectant mothers can book pregnancy scans, including the Early Pregnancy Scan for initial confirmation and the NT-NB Scan specifically designed for twin pregnancies to determine chorionicity, cashlessly through Visit Health's diagnostic network. Results are tracked digitally through the app, keeping the full scan history organized as appointments become more frequent through the pregnancy.

Teleconsultation: Gynaecology and Obstetric Specialists

Questions about chorionicity results, nutrition targets, or any of the warning signs above deserve a direct conversation with a specialist rather than piecing together information from different sources. Visit Health's teleconsultation platform connects employees with gynaecologists and obstetric specialists across 30-plus specialties, with general physician consultations typically available within 15 minutes and specialist consultations within 60 minutes.

AI-Powered Smart Reports

A chorionicity scan report describing lambda or T-signs, or a growth scan comparing two sets of measurements, isn't always straightforward to interpret on your own. Visit Health's Smart Reports translate these findings into plain-language summaries, explaining what type of twin pregnancy is involved and what that means for the monitoring schedule ahead.

What are the earliest signs of a twin pregnancy?

Early twin pregnancy symptoms often include more intense morning sickness, deeper fatigue, faster early weight gain, and a uterus measuring larger than expected, though these overlap heavily with typical singleton symptoms.

Can you feel different symptoms with twins before a scan confirms it?

Some people notice more intense symptoms or earlier fetal movement, but symptoms alone aren't reliable. Signs of twins can raise suspicion, but an ultrasound is the only way to confirm.

When can an ultrasound detect twins?

Twins can typically be seen on ultrasound from around 6 weeks onward, with the standard dating scan around 8 weeks usually confirming the number of babies.

What is chorionicity and why does it matter?

Chorionicity refers to how many placentas twins have, determined on a twin pregnancy scan between 10 and 14 weeks; it directly affects how often you're monitored and what complications your care team watches for.

Do you need to eat for three with twins?

Not literally. Calorie needs rise by about 40% on average, but the exact target depends on your pre-pregnancy weight and health, which is why twin pregnancy care often includes individualized nutrition guidance.

How often are prenatal visits with twins?

Visits are typically more frequent than in a singleton pregnancy, especially in the third trimester, with monochorionic twins usually scanned every two weeks and dichorionic twins every four.

Are twin pregnancies considered high risk?

Yes, twin pregnancies are generally classified as higher risk due to increased chances of preterm labor, preeclampsia, and growth differences between babies, which is why closer monitoring is standard.

When do most twin pregnancies deliver?

More than half of twin pregnancies deliver before 37 weeks, and monochorionic-monoamniotic twins are often delivered even earlier, around 32–34 weeks, as a precaution.
