The urease test detects the urease enzyme produced by bacteria. It's used clinically to identify Helicobacter pylori in gastric biopsies or urea breath tests and to detect urease-producing organisms (e.g., Proteus species) in urinary infections and kidney stones. Urease hydrolyzes urea into ammonia and carbon dioxide, raising pH and producing a color change in media or indicators to confirm activity.

A positive urease test indicates presence of urease-producing bacteria. In gastric testing it strongly suggests Helicobacter pylori colonization, associated with gastritis, peptic ulcers and increased stomach cancer risk. In urine or wound samples it points to organisms such as Proteus. The test detects urease activity (urea → ammonia) causing a pH change; a positive result guides targeted antibiotic treatment.

The urease test performed during endoscopy (rapid urease test) detects Helicobacter pylori on a gastric biopsy. A small mucosal sample is placed in urea-containing medium with a pH indicator; bacterial urease hydrolyzes urea to ammonia, raising pH and changing the medium’s color. It gives rapid results (minutes–hours) and reliably confirms H. pylori, though acid-suppressing drugs or antibiotics can reduce accuracy.

The urease test detects Helicobacter pylori by identifying its urease enzyme, which breaks down urea into ammonia and carbon dioxide, raising local pH. Urease is an enzyme that breaks down urea into ammonia and carbon dioxide. It’s useful ecologically for nitrogen cycling and in some diagnostic tests and industrial processes. However, when produced by pathogenic bacteria it raises local pH, promoting kidney stones, urinary tract infections and gastric damage. Thus urease is context‑dependent—beneficial in environmental and applied settings but harmful in human infections.

H. pylori negative means diagnostic tests show no evidence of Helicobacter pylori infection. Common tests include urea breath, stool antigen, blood antibody and gastric biopsy/rapid urease testing. A negative result suggests H. pylori is not causing gastritis or peptic ulcer disease, but false negatives can occur after recent antibiotics, bismuth, or proton‑pump inhibitors; retesting is advised if symptoms persist.
