Spot urine microalbumin is a one-time urine test that measures small amounts of albumin (protein) relative to creatinine to detect early kidney damage, especially in diabetes or high blood pressure. A urine albumin-to-creatinine ratio under 30 mg/g is normal; 30–300 mg/g indicates microalbuminuria (early kidney disease), and above 300 mg/g suggests more advanced proteinuria, guiding monitoring and treatment.

A normal spot urine albumin-to-creatinine ratio (ACR) is below 30 mg/g (about <3 mg/mmol). Results of 30–300 mg/g indicate moderately increased albumin (microalbuminuria), while values above 300 mg/g indicate severely increased albumin (macroalbuminuria). ACR from a random urine sample is used to screen for kidney damage; abnormal results usually lead to repeat testing and further evaluation.

High microalbumin (microalbuminuria) means small amounts of albumin in urine, often the earliest sign of kidney damage. It’s commonly linked to diabetes and high blood pressure and signals increased risk of chronic kidney disease and cardiovascular events. Confirm with repeat albumin-to‑creatinine ratio tests. Management focuses on blood sugar and blood pressure control (often ACE inhibitors/ARBs), lifestyle changes, monitoring, or nephrology referral if persistent.

If a spot urine test shows high protein, it may be transient (from exercise, fever, dehydration) or signal kidney damage from diabetes, hypertension, or other causes. Repeat testing—ideally a first‑morning albumin‑to‑creatinine ratio or 24‑hour collection—is recommended, along with blood tests for kidney function. Your clinician may order urine microscopy, imaging, or nephrology referral to identify cause and start treatment.

Lower urine microalbumin by controlling blood pressure and blood glucose—use ACE inhibitors or ARBs if prescribed—while cutting dietary salt and excess protein. Lose weight, exercise regularly, quit smoking, limit alcohol, manage cholesterol, treat infections, and avoid NSAIDs and other nephrotoxins. Repeat testing and close follow-up with your healthcare provider will guide medication adjustments and lifestyle changes to reduce albuminuria.

A high spot urine creatinine usually reflects concentrated urine from dehydration, recent high‑protein/meat intake, intense exercise, or greater muscle mass. It can also occur with some medications. Persistently high levels, or abnormal serum creatinine/proteinuria, warrant further evaluation (repeat testing, 24‑hour urine or serum creatinine and eGFR) by your clinician. Drink fluids and follow up if concerned.
