Protocol: perform urgent brain MRI as soon as possible after suspected stroke. Core sequences: diffusion-weighted imaging (DWI) with ADC maps to detect acute ischemia; T2/FLAIR to age lesions; GRE/SWI to detect hemorrhage or microbleeds; MR angiography (TOF or contrast) to identify vessel occlusion; perfusion imaging to assess penumbra. Screen for MRI contraindications, monitor vitals, and check renal function if contrast is used.

An MRI brain stroke protocol usually takes fifteen to thirty minutes. A rapid acute-stroke exam (diffusion-weighted imaging, ADC, FLAIR) can be completed in about ten to fifteen minutes. More comprehensive studies that add MR angiography and perfusion imaging or require contrast may extend the scan to thirty-five to forty-five minutes. Total time varies with scanner, sequences ordered and patient cooperation.

Recognize stroke signs quickly (Face droop, Arm weakness, Speech difficulty — Time to act) and call emergency services immediately. EMS secures airway/breathing, rapid transport and urgent brain imaging (CT/MRI) to distinguish ischemic vs hemorrhagic stroke. Eligible ischemic strokes may receive IV thrombolysis and/or endovascular thrombectomy. Hospital care includes blood pressure and glucose control, neuro-monitoring, and admission to a stroke unit for rehabilitation.

An MRI should be done as soon as possible after stroke onset—diffusion‑weighted MRI can detect ischemic injury within minutes and is ideal for acute diagnosis. If reperfusion therapy is being considered, urgent brain imaging (often CT first) must not be delayed; MRI is appropriate immediately if available without delaying treatment. Otherwise aim to perform MRI within 24 hours, and certainly within 48 hours for confirmation and planning.

Diffusion-weighted MRI (DWI) is the most sensitive and specific MRI for detecting acute ischemic stroke, identifying infarcts within minutes. For comprehensive evaluation, DWI is typically combined with apparent diffusion coefficient maps, perfusion-weighted imaging to assess the penumbra, MR angiography to visualize vessels, and susceptibility-weighted imaging for hemorrhage or microbleeds. This multimodal MRI guides treatment decisions.

The stroke protocol: recognize symptoms (sudden face droop, arm weakness, speech difficulty), call emergency services immediately. On arrival: rapid clinical assessment, CT/MRI to distinguish ischemic vs hemorrhagic stroke, blood tests and ECG. If ischemic and eligible, give IV thrombolysis (within the treatment window) and consider mechanical thrombectomy for large-vessel occlusion. Admit to a stroke unit and begin early rehabilitation.
