An MRI CSF flow study (cine or phase‑contrast MRI) is a noninvasive scan that visualizes and quantifies cerebrospinal fluid movement over the cardiac cycle. It measures flow direction, velocity, and stroke volume at sites like the aqueduct and foramen magnum to help diagnose hydrocephalus, normal‑pressure hydrocephalus, CSF leaks, and syringomyelia. It uses no ionizing radiation and can guide management decisions.

CSF stands for cerebrospinal fluid. In MRI, CSF is the clear fluid surrounding the brain and spinal cord; it cushions, supplies nutrients, and removes waste. On MRI, CSF appears hypointense on T1-weighted images and hyperintense on T2-weighted images. Evaluation of CSF spaces helps detect conditions such as hydrocephalus, hemorrhage, infection, and inflammatory or degenerative disorders.

The cerebrospinal fluid (CSF) study collects and analyzes fluid around the brain and spinal cord to diagnose and monitor conditions such as infections (meningitis, encephalitis), bleeding (subarachnoid hemorrhage), inflammatory or autoimmune disorders (multiple sclerosis), and cancer involvement. It assesses cell counts, protein, glucose, microbes, and biomarkers, measures opening pressure, and helps guide treatment decisions and prognosis.

Abnormal cerebrospinal fluid (CSF) flow can cause persistent headache (often worse on waking), nausea or vomiting, blurred or double vision, balance and gait problems, cognitive slowing or confusion, excessive sleepiness or irritability, urinary incontinence, pulsatile tinnitus, neck pain/stiffness, and in infants a rapidly increasing head size or bulging fontanelle. Sudden worsening may signal shunt malfunction or acute obstruction.

If a cerebrospinal fluid (CSF) test is positive, it indicates abnormal findings—such as infection, inflammation, bleeding, or malignant cells. Management depends on the cause: urgent hospitalization when infectious or hemorrhagic, targeted antimicrobials or antivirals, steroids for inflammatory causes, oncology referral for cancer, and further imaging, repeat CSF testing and specialist consultation. Close monitoring and supportive care follow.

If CSF flow is blocked, cerebrospinal fluid accumulates, raising intracranial pressure and enlarging brain ventricles (hydrocephalus). Symptoms include severe headache, nausea, vomiting, blurred vision or papilledema, altered consciousness, seizures, cognitive and gait disturbances, and urinary problems. Acute obstruction can cause rapid deterioration and brain herniation. Treatment aims to relieve the blockage and lower pressure, often with surgical shunting or endoscopic procedures.
