MRI Scan Brain Seizure Protocol, in Visit Clinic

A brain MRI with seizure protocol is a specialized MRI study optimized to detect structural causes of seizures. It uses high-resolution, thin-slice and targeted sequences (T1, T2, FLAIR, DWI, SWI/gradient-echo, sometimes contrast) and oblique coronal views of the hippocampi to identify tumors, cortical dysplasia, hippocampal sclerosis, vascular lesions, or scarring that may underlie epilepsy and guide treatment or surgery.

A brain MRI for seizures looks for structural abnormalities that can cause or result from seizures — tumors, stroke or hemorrhage, cortical dysplasia, hippocampal (mesial temporal) sclerosis, vascular malformations, infections, inflammation, or scarring (gliosis). It can show acute seizure-related changes on specific sequences, but may be normal in many epilepsy cases. High-resolution epilepsy MRI protocols increase detection of subtle lesions.

Epilepsy-protocol brain MRI is usually performed without routine contrast, using high-resolution 3T sequences (thin-slice T1, T2, FLAIR, volumetric imaging) optimized to detect cortical dysplasia, hippocampal sclerosis and subtle lesions. Gadolinium contrast is reserved for specific indications—suspected tumor, infection, inflammation, postoperative evaluation, seizure-focus localization, or when non-contrast MRI is inconclusive—based on clinical judgment.

MRI is commonly used and generally safe for people with seizures to identify structural causes. Scans may require planning: uncontrolled seizures, recent seizure activity, metallic implants or certain neurostimulators can affect safety—some devices are MRI‑conditional only. You may need monitoring, EEG correlation, sedation, or an open MRI for comfort. Always inform the imaging team and your neurologist before the scan.

The first step in seizure protocol is to stay calm and protect the person from injury. Clear nearby hazards, cushion their head, and loosen tight clothing while avoiding restraints and not placing anything in the mouth. Time the seizure and monitor breathing. Call emergency services if it lasts longer than five minutes, they have difficulty breathing, it’s their first seizure, or they’re injured.

Best MRI for epilepsy is a dedicated high-resolution epilepsy protocol on a 3T scanner, including thin-slice coronal and axial T1-weighted 3D volumetric (e.g., MPRAGE), T2-weighted and FLAIR sequences—especially thin-slice coronal FLAIR/T2 through the hippocampi—to detect mesial temporal sclerosis. Susceptibility-weighted or gradient-echo sequences help identify cavernomas or microbleeds. Contrast is used selectively.
