Yes — many MRI scans are performed without contrast. Contrast agents (usually gadolinium-based) are used only when extra detail is needed, such as evaluating tumors, infection, inflammation, or blood vessels. Whether contrast is used depends on the clinical question, imaging protocol and kidney function. Risks are uncommon but include allergic reactions and, rarely in severe renal impairment, nephrogenic systemic fibrosis.

MRI enteroclysis is a specialized MRI of the small intestine in which contrast fluid is introduced directly into the small bowel via a thin tube passed through the nose or mouth to distend loops and produce high-resolution images. It helps evaluate Crohn’s disease, strictures, tumors and obstructions, offers excellent soft-tissue detail without ionizing radiation, and may cause temporary discomfort.

An MRI angiogram of the head without contrast is a noninvasive scan that uses magnetic fields and radio waves (time-of-flight or phase‑contrast techniques) to image cerebral arteries and veins without gadolinium. It detects aneurysms, vessel narrowing, or malformations, is safer for people with kidney problems or contrast allergies, and is less sensitive than contrast studies for very small or slow-flow vessels.

Prepare by following fasting instructions (usually nil by mouth for 4–6 hours) and any bowel‑cleansing regimen your provider prescribes. You may need to avoid iron supplements and wear metal‑free clothing; bring prior imaging and inform staff about pregnancy, allergies, kidney disease, or implants. A nasojejunal tube will be placed for contrast, and you may receive an antispasmodic and IV contrast during the scan.

Many tumors can be seen on MRI without contrast using T1, T2 and FLAIR sequences, which detect mass effect, edema and signal changes. However, gadolinium contrast increases sensitivity and helps define tumor margins, detect small or enhancing lesions, and distinguish tumor from scar or inflammation. Some lesions may only be apparent with contrast, so contrast-enhanced scans are recommended when safe and clinically indicated.

An MRI without contrast is generally safe. Common effects include loud noise, transient claustrophobia or anxiety, and discomfort from lying still. Some people report temporary tingling, warmth, dizziness or headache. Rarely, metallic implants or fragments can heat, move, or malfunction, causing burns or device problems; pregnancy and certain implants require precaution. Long-term side effects are not known.
