Methadone is a long-acting opioid used to treat opioid use disorder and severe chronic pain. In addiction treatment it reduces withdrawal symptoms and cravings and stabilizes patients as part of medication-assisted therapy, often under supervision. It can also be prescribed for persistent cancer or noncancer pain when other treatments fail, but requires careful dosing because of overdose and interaction risks.

Because methadone has a long, variable half‑life and accumulates, effects and toxicity can be delayed; slower titration and close monitoring reduce risk of respiratory depression, overdose and harmful interactions. Methadone, an opioid, can cause drowsiness and sedation, especially when you start therapy, after dose increases, or when combined with alcohol, benzodiazepines, or other sedating drugs. It may impair concentration and driving and, in some people, cause dangerous respiratory depression. Tell your prescriber about other medicines and seek medical advice promptly if you experience excessive sleepiness, confusion, or breathing difficulties.

Yes. Methadone can rarely cause hallucinations (visual or auditory), particularly at high doses, with overdose, in older people or those with kidney/liver impairment, or when combined with other CNS depressants or serotonergic drugs. Hallucinations may indicate toxicity, serotonin syndrome, or withdrawal/overdose. Contact your prescriber or seek urgent care if hallucinations, confusion, breathing problems, or other concerning symptoms occur.
