CD4 Absolute Count, in Visit Clinic

Absolute CD4 count is the number of CD4 T lymphocytes per microliter of blood; it measures immune system strength and is used to monitor HIV progression and guide treatment decisions and prophylaxis against opportunistic infections. It differs from CD4 percentage. In adults, normal counts are roughly 500–1,600 cells/µL; counts below 200 cells/µL indicate high risk for AIDS‑defining infections.

A low CD4 count indicates weakened immune defenses, increasing risk of opportunistic infections, certain cancers, and faster progression of HIV. Symptoms can include recurrent infections, fever, weight loss, night sweats, and prolonged diarrhea. Low counts often prompt antiretroviral therapy adjustments and preventive (prophylactic) treatments to reduce infection risk and improve immune recovery; close medical monitoring is essential.

Low CD4 counts occur most commonly with HIV infection. Other causes include chronic viral infections (CMV, EBV, HTLV‑1), active tuberculosis and other severe bacterial or fungal infections, hematologic malignancies (lymphoma, leukemia), primary immunodeficiencies (SCID, DiGeorge), autoimmune diseases (SLE), immunosuppressive drugs, chemotherapy, radiation, severe malnutrition, bone‑marrow failure, and idiopathic CD4 lymphocytopenia.

CD4 stands for Cluster of Differentiation 4. It is a glycoprotein expressed on the surface of immune cells—primarily helper T lymphocytes, as well as monocytes, macrophages and dendritic cells—and functions as a co-receptor for antigen recognition with MHC class II molecules. CD4 count is a key laboratory marker used to assess immune status, especially in HIV infection.

The fastest, evidence‑based way to raise CD4 is to start and strictly adhere to antiretroviral therapy and follow your HIV clinician’s plan (regular CD4/viral‑load monitoring and treatment of co‑infections). Support immune recovery with good nutrition, regular exercise, adequate sleep, vaccination as advised, quitting smoking and avoiding alcohol or illicit drugs. Consult your healthcare provider promptly for individualized care.

Low CD4 counts (especially <200 cells/µL) greatly raise the risk of opportunistic infections and shorten life expectancy if untreated—sometimes to months or a few years depending on complications. However, with timely, effective antiretroviral therapy many people recover CD4 counts and can achieve near-normal life expectancy, though a small long-term excess risk may remain. Urgent medical care and ART are essential.
