A positive beta-2 glycoprotein IgG means you have antiphospholipid antibodies, which raises the risk of blood clots (venous and arterial), stroke and pregnancy complications such as recurrent miscarriage. A single positive result can be transient; repeat testing after at least 12 weeks is needed to confirm persistence. Diagnosis and treatment depend on clinical history and additional tests (e.g., lupus anticoagulant, anticardiolipin).

Most laboratories consider beta‑2 glycoprotein I (β2‑GPI) IgG levels under 20 units (often reported as GPL or U) to be normal/negative. Levels of 20–40 are typically borderline/indeterminate, while values above about 40 GPL (some labs use >20 as a positive cutoff) are regarded as positive and may prompt further clinical evaluation. Cutoffs can vary by lab.

High anti–beta2‑glycoprotein I antibody levels most commonly indicate antiphospholipid antibodies, seen in antiphospholipid syndrome and other autoimmune conditions (eg, lupus). Levels can also rise transiently after infections, with certain drugs, or rarely in healthy people. The beta‑2 glycoprotein I (β2‑GPI) antibody test detects autoantibodies directed against β2‑glycoprotein I, one of the antiphospholipid antibodies. It’s used to help diagnose antiphospholipid syndrome in people with unexplained blood clots, recurrent miscarriages or low platelets. Labs measure IgG and IgM isotypes; persistent positivity on repeat testing (≥12 weeks) indicates higher clotting risk and guides management.

Beta‑2 glycoprotein I (β2GPI) is the principal autoantigen in antiphospholipid syndrome (APS), linked to recurrent arterial and venous thrombosis, pregnancy loss and pre‑eclampsia, stroke, thrombocytopenia, livedo reticularis, heart valve lesions and catastrophic APS. Anti‑β2GPI antibodies are also frequently seen in systemic lupus erythematosus (SLE) and increase thrombotic and obstetric risk in affected patients.

A lupus anticoagulant test is a blood test that detects antiphospholipid antibodies which interfere with clotting tests and are associated with increased risk of abnormal blood clots and pregnancy complications. It uses specialized coagulation assays (for example dRVVT and mixing studies) and requires confirmation with repeat testing at least 12 weeks later to diagnose antiphospholipid syndrome and guide anticoagulation decisions.
