A prostatic acid phosphatase (PAP) test is a blood test that measures levels of an enzyme produced by prostate cells. Historically used to detect and monitor prostate cancer, elevated PAP can indicate prostate cancer, prostatitis, benign enlargement, or metastasis. It is now largely replaced by prostate‑specific antigen (PSA) but may still help assess advanced or metastatic disease and treatment response.

ACP (Advance Care Planning) in prostate cancer is the process of discussing, recording and reviewing a patient’s preferences for future care and treatment — including choices about treatments, resuscitation, preferred place of care, and appointing a healthcare proxy or power of attorney. It ensures care aligns with the patient’s values if they can’t decide, and should be started early and updated as needs change.

Prostatic acid phosphatase rises in prostate cancer because malignant prostate cells produce and secrete more of this enzyme, and tumor growth with tissue disruption releases intracellular enzyme into the bloodstream. Levels correlate with tumor burden and are particularly elevated with bone metastases, where cancer invasion increases enzyme release. Historically, PAP was used as a marker of advanced or metastatic prostate disease.

High acid phosphatase means elevated levels of this enzyme in blood and often signals tissue damage. In men it commonly points to prostate disease (benign enlargement or prostate cancer), especially with metastasis. It can also rise with bone disorders (fracture, Paget’s disease), liver disease, certain infections or hematologic conditions. It’s non‑specific—your doctor will order targeted tests (PSA, imaging, liver and bone studies) to find the cause.

Acid phosphatase test measures levels of the enzyme acid phosphatase in blood (and sometimes other fluids). It’s used mainly to detect elevated prostatic acid phosphatase linked to prostate disease—historically for diagnosing and monitoring prostate cancer and metastases—and can also rise with certain bone, liver, or blood disorders. It’s used in forensic semen detection and to evaluate some metabolic conditions.

Alkaline phosphatase (ALP) normal values for adults typically range about 30–130 U/L, though reported lab ranges often span roughly 40–150 IU/L. Reference limits vary by laboratory method, age and sex; children and pregnant women commonly have higher values. Abnormal results warrant clinical correlation and follow-up testing by your healthcare provider.
