X Ray Whole Spine AP View

An AP (anteroposterior) view of the spine is an X‑ray projection taken with the beam entering the front and exiting the back of the patient. It shows vertebral bodies, disc spaces and overall alignment, useful for detecting fractures, deformity, infection or hardware position. Typically performed standing or supine with arms out of the field; posterior elements are less visible, so a lateral view is often added.

A full spine X‑ray is called a full‑spine (whole‑spine) radiograph or spine survey, often referred to as a scoliosis series when assessing curvature. It typically includes standing anteroposterior (AP) and lateral long‑cassette views to visualize the entire cervical, thoracic, and lumbar spine for alignment, deformity, trauma, or degenerative changes.

AP view means anteroposterior projection: the X‑ray beam passes from the patient's front (anterior) to back (posterior) onto the detector. It’s commonly used for supine, bed‑bound, or trauma patients when a standard PA view isn’t possible. Compared with PA films, AP images can magnify the heart and mediastinum and often have reduced image quality, so findings are interpreted with those limitations in mind.

Normal anteroposterior (AP) diameter of the lumbar spinal canal at L4–L5 is generally ≥13 mm, with typical measured values often in the ~15–25 mm range. An AP diameter of 10–13 mm is considered relative narrowing (possible stenosis), while <10 mm is commonly regarded as absolute spinal stenosis requiring further evaluation.

The best imaging depends on the suspected spine problem. MRI is the preferred study for most spinal complaints because it shows discs, nerve roots, spinal cord, and soft tissues. CT better defines bone detail and is used for fractures or when MRI is contraindicated. Plain X-rays assess alignment and chronic degenerative change. Nuclear bone scans can help detect infection or metastasis in specific cases.
