An oblique wrist X-ray provides an angled view that complements AP and lateral images to better visualize carpal bones, distal radius and ulna, and radiocarpal joint spaces. It helps detect fractures, subtle scaphoid or carpal injuries, dislocations, and degenerative changes that may be missed on other views, and aids assessment of bone alignment and joint integrity after trauma or with persistent wrist pain.

An oblique X-ray is a radiographic view taken at an angle between the standard front (AP) and side (lateral) positions. Angled projections better reveal bones, joints, and fractures that are hidden on straight views—commonly used for spine, ribs, facial bones, and extremities. It provides a complementary perspective to improve detection of alignment issues, occult fractures, and subtle joint abnormalities.

For an oblique hand X‑ray, the patient places the hand on the image receptor with the palm down, then externally rotate the hand about 45° into a semi‑pronated oblique. Fingers are extended and slightly separated; the thumb may be abducted. The central ray is perpendicular to the receptor and is typically centered at the third metacarpophalangeal joint.

Position the patient so the target region is rotated obliquely (commonly 45°) relative to the detector; choose anterior or posterior oblique per projection. Center the X‑ray beam, collimate tightly, and set appropriate exposure factors. Use lead shielding and instruct the patient to hold still or hold breath as required. Mark side and ensure the study is performed by trained personnel following institutional radiation‑safety and imaging protocols.

The oblique muscles (external and internal) support trunk movement and stability: they rotate and laterally flex the spine, help flex the torso, compress abdominal contents to support posture and intra‑abdominal pressure, assist forced expiration and coughing, and protect internal organs. They also contribute to pelvic stability and coordinated core function during lifting and athletic movements.

A wrist X-ray is usually painless. The X-ray procedure itself does not cause pain, though holding positions or pressure on an already injured, swollen, or fractured wrist can be uncomfortable or painful. You may feel brief pressure when a technician repositions your hand. The scan is quick, and any discomfort is short-lived; tell staff about severe pain so they can adjust.
