The Y view (scapular Y) is an oblique shoulder X‑ray showing the scapula in profile so the acromion, coracoid process and scapular spine form a Y. It assesses the humeral head’s relation to the glenoid to detect anterior or posterior dislocation and proximal humeral fractures. It’s obtained with the scapular body perpendicular to the image and complements AP shoulder views.

Three standard shoulder X‑ray views are: AP (anteroposterior) view — often obtained in external or neutral rotation to assess joint space, humeral head, and glenoid; axillary (axillary lateral) view — evaluates glenohumeral alignment and dislocations; scapular Y (lateral scapula) view — useful for assessing anterior/posterior displacement and proximal humerus or scapular fractures.

The scapular Y (Y‑view) radiograph helps assess shoulder alignment and can reveal displaced proximal humerus, glenoid, or scapular fractures and dislocations. However, it may miss nondisplaced or subtle fractures and provides limited detail for complex injuries. If clinical suspicion remains or precise fracture characterization or surgical planning is needed, additional views (axillary/AP) or CT are recommended.

A scapular Y‑view X‑ray is reliable for showing shoulder alignment and diagnosing major dislocations or gross fractures, with good accuracy for detecting displacement. However, it has limited sensitivity for small or occult fractures, intra‑articular fragments and soft‑tissue injuries. Positioning and image quality affect accuracy. If radiographs are inconclusive or symptoms persist, CT or MRI is recommended for more sensitive evaluation.

Wear a loose, metal-free top or be prepared to change into a hospital gown for a scapular Y view. Remove bras with underwires, necklaces, piercings, and clothing with zippers or buttons over the shoulder. Keep the area bare so the technologist can position you comfortably; let staff know if you’re pregnant or in pain. Comfortable, easily removable clothing is best.

The scapular Y view reveals humeral head position and glenohumeral alignment, making it useful for detecting anterior or posterior shoulder dislocations. It also shows fractures of the scapular body, neck, acromion, coracoid process, and glenoid rim, and can demonstrate proximal humerus fractures and significant displacement or comminution. It’s used to assess joint congruity and fracture alignment not seen on standard AP shoulder films.
