X Ray PNS Water/OM View

X-ray PNS Waters view is a radiographic projection to assess the paranasal sinuses, especially the maxillary and anterior ethmoid sinuses. With the patient's head extended and the X-ray beam angled, it highlights sinus air-fluid levels, mucosal thickening and facial bone fractures. It's a quick, low-cost test often used when CT is unnecessary or unavailable to evaluate sinusitis or trauma.

The best single view for paranasal sinus (PNS) X‑ray is the Waters' (occipitomental) view. It optimally displays the maxillary sinuses, shows air–fluid levels and sinus opacification with minimal overlapping of facial bones. Lateral and Caldwell (PA) views can be used as complementary projections to evaluate sphenoid, frontal and ethmoid sinuses or when more detail is required.

Position patient upright facing the image receptor. Extend the neck so the mentomeatal line is perpendicular to the receptor (placing the orbitomeatal line ~37° to the receptor). Center the image at the acanthion. Direct the central X‑ray beam perpendicular to the receptor, exiting at the acanthion. Keep the head midline, mouth closed, immobilize, and use appropriate sinus/cranial exposure settings.

A PNS test (paranasal sinuses X‑ray) assesses the air‑filled sinuses around the nose. It’s used to evaluate sinus infections, inflammation, mucosal thickening, fluid levels, suspected sinus fractures or chronic sinusitis, and to guide treatment. It’s a quick, low‑cost imaging test but may be less sensitive than CT for complex cases or complications; further imaging is sometimes needed.

Doctors diagnose a sinus infection mainly from medical history and a physical exam, assessing symptom type and duration (purulent nasal discharge, facial pain/pressure, nasal blockage, fever). Bacterial infection is suspected when symptoms last beyond 10 days or worsen after initial improvement. Nasal endoscopy can visualize secretions; CT imaging is reserved for chronic, severe or complicated cases. Sinus cultures are rarely required.

A paranasal sinus (PNS) X‑ray is quick: the actual exposures last only a few seconds, and the whole procedure usually takes about 10–20 minutes including positioning and multiple views. Busy clinics or extra views can extend this to around 30 minutes. Preparation is minimal (remove jewelry); results may be available the same day or within a few days depending on the facility.
