X Ray Pelvis Ferguson View

The Ferguson view of the pelvis is an AP axial radiographic projection with a cephalad-angulated central ray (about 30°) aimed at the lumbosacral junction. It opens the L5–S1 interspace and better demonstrates the sacrum, sacroiliac joints, and lumbosacral alignment for assessing trauma, degenerative change, or alignment abnormalities. Patient may be supine or upright per clinical need.

Pelvic radiography typically includes an AP pelvis (supine) and AP bilateral hips. In trauma, an extended pelvic series adds inlet and outlet views to assess pelvic ring displacement, plus Judet (oblique) views — iliac and obturator oblique — for acetabular fractures. For hip-focused imaging, a frog‑leg lateral or cross‑table lateral may be added to visualize femoral head and neck.

The modified Ferguson (AP axial lumbosacral) view is taken with the patient supine, legs extended, and the central ray angled cephalad (about 30–35°) toward the lumbosacral junction. The beam is centered at the L5–S1 region (roughly 1.5 inches above the pubic symphysis) to open the L5–S1 disc space and demonstrate lumbosacral alignment.

A pelvic X-ray shows the bony pelvis—ilium, ischium, pubis—plus sacrum, coccyx, both hip joints and the top of the femurs. It detects fractures, dislocations, degenerative changes (arthritis), joint-space narrowing, bone lesions, calcifications, implants (hip replacements) and major alignment or leg‑length differences. Soft-tissue detail is limited; CT or MRI may be needed for complex injuries.

Sclerosis of the sacroiliac (SI) joint is an imaging finding where bone near the joint becomes abnormally dense. It commonly reflects chronic stress, degenerative osteoarthritis or long‑standing inflammation (for example spondyloarthritis). Symptoms can include lower‑back or buttock pain, stiffness and reduced mobility. Treatment targets pain relief, physical therapy, possible steroid injections, and management of any underlying inflammatory condition.

The Dunn view of the pelvis is a specific radiographic hip projection that profiles the femoral head–neck junction. It is obtained with the patient supine, the hip flexed to about 45° and slightly abducted, producing an oblique lateral image. This view improves detection of femoroacetabular impingement, cam deformities and slipped capital femoral epiphysis by better showing anterior head–neck morphology.
