X RAY LUMBAR SPINE OBLIQUE VIEW

The oblique view of the lumbar spine is an X-ray taken with the patient rotated about 45° to profile the vertebrae. It best visualizes the zygapophyseal (facet) joints and pars interarticularis — useful for detecting spondylolysis, facet arthropathy, and certain fractures. The projection produces the "Scottie dog" silhouette, allowing assessment of alignment and bony defects not seen on AP or lateral views.

An oblique view X‑ray is a radiographic projection taken with the body or film rotated between the frontal (AP) and side (lateral) positions to image structures that overlap on standard views. It improves visualization of bones, joints, spine, ribs and facial bones, helping detect fractures, dislocations, or spinal foraminal narrowing. The angle varies (often about 45°) depending on the area examined.

The four standard lumbar spine radiographic views are: anteroposterior (AP), lateral, and two oblique views (left and right). AP shows vertebral alignment and disc spaces; lateral visualizes vertebral bodies, discs, and the spinal canal; obliques assess facet joints and pars interarticularis. Together they evaluate alignment, fractures, degenerative change, and spondylolysis. They’re used for initial assessment of back pain.

They improve detection of pars defects (spondylolysis), subtle fractures, facet arthropathy and foraminal narrowing causing nerve-root impingement. Obliques provide complementary alignment and bony detail, helping guide diagnosis, management and decisions about further imaging.
