The Y view is an oblique shoulder radiograph in which the scapula appears as a Y formed by the acromion, coracoid process and scapular body. It's obtained with the patient rotated so the scapula is lateral, and it shows the relationship of the humeral head to the glenoid. It's used to detect anterior or posterior dislocation and to evaluate scapular and proximal humeral fractures.

Position the patient upright (or prone) with the affected shoulder against the image receptor. Rotate the torso 45–60° so the scapular body is perpendicular to the IR; place the arm across the chest. Direct the central ray perpendicular to the scapulohumeral joint, center on the scapular body/acromion, and use suspended respiration. The Y view shows acromion, coracoid and scapular body for dislocation/fracture assessment.

The scapular Y (outlet) shoulder radiograph is reliable for detecting gross humeral head displacement, anterior/posterior dislocations and obvious scapular or proximal humeral fractures when positioned correctly. However, its sensitivity for nondisplaced fractures, small intra‑articular fragments and soft‑tissue injuries is limited. If clinical suspicion remains despite a normal Y view, cross‑sectional imaging (CT or MRI) provides higher accuracy.

For a scapular Y view, wear a hospital gown or loose clothing without metal fasteners. Remove bras, jewelry, necklaces, piercings, and any metal objects from the shoulder, chest and neck area. Avoid shirts with zippers or buttons; sports bras may need removal. Dress comfortably and follow the radiographer’s instructions for positioning to ensure a clear image.
