The axial (axillary) shoulder X-ray is an inferosuperior projection obtained with the arm abducted and the beam directed through the axilla. It visualizes the glenohumeral joint—humeral head, glenoid rim, coracoid and acromion—helping detect anterior/posterior dislocations, glenoid fractures, and fragment displacement. It complements AP and scapular Y views for comprehensive assessment.

An AP (anteroposterior) view of the shoulder is a frontal X‑ray taken with the beam passing from front to back. It visualizes the clavicle, acromion, glenoid and proximal humerus to assess fractures, dislocations, joint space and alignment. It can be obtained upright or supine, often with neutral, internal or external rotation to highlight different structures.

The three standard shoulder X‑ray views are: AP (anteroposterior) — evaluates the glenohumeral joint, humeral head and proximal humerus; Axillary — provides an axial view to detect anterior/posterior dislocations and assess glenoid rim; Scapular Y (lateral) — a lateral scapular projection used to confirm humeral head position relative to the glenoid and scapular alignment.

The shoulder axillary view is an X‑ray projection taken through the armpit with the arm abducted (typically 70–90°) to visualize the glenohumeral joint from below. It clearly shows the relationship of the humeral head and glenoid, helping detect dislocations, fractures, and joint space abnormalities. It’s often used alongside other views when standard projections are inconclusive.

A shoulder axial (axillary) X‑ray itself is not painful, but obtaining the view may cause discomfort because it requires arm positioning or abduction. Patients with acute injury, fractures, dislocation, or limited mobility often experience pain during positioning. Radiographers use gentle technique, cushions, and alternative projections (modified axial or Velpeau) to reduce discomfort. Tell staff about pain so they can adapt the exam.

An axial X‑ray (axial view) is a radiographic image taken perpendicular to the body's long axis, creating a transverse (cross‑sectional) projection of anatomy. It helps assess bone alignment, fractures, joint spaces and certain skull or chest structures. Clinicians use axial views with other projections or CT to better evaluate injuries, structural abnormalities and foreign bodies.
