The lateral hand X-ray is a side projection taken with the hand positioned true lateral (ulnar aspect on the detector or with fingers fanned) so phalanges and metacarpals are seen in profile. It demonstrates dorsovolar displacement, angulation, and alignment of phalanges, metacarpals, and wrist, helping detect fractures, dislocations, joint-space changes, and soft-tissue swelling not seen on AP views.

A lateral view X‑ray is a side‑to‑side radiographic projection obtained with the X‑ray beam passing through the body from one lateral side to the other, perpendicular to the frontal (AP/PA) view. It helps localize lesions in depth, evaluate alignment (spine), and visualize structures obscured on frontal films. The patient stands or lies with the side of interest against the detector and arms positioned out of the field.

For standard hand X-rays, place the palm flat on the detector with the wrist and forearm on the table, fingers fully extended and slightly spread, and the hand centered over the film (third metacarpal). Ensure no rotation and keep the wrist neutral. Obtain additional views as needed: oblique with the hand rotated about 45° and lateral with the fingers superimposed.

A bilateral hand X‑ray is an imaging test that takes radiographic images of both hands and wrists simultaneously. It evaluates bones, joints and soft‑tissue shadows to detect fractures, dislocations, arthritis, growth‑plate injuries, infections, or congenital abnormalities. Often used for comparison between sides, it helps guide diagnosis, treatment planning and monitoring. Radiation exposure is low and the procedure is quick and noninvasive.

A routine hand X‑ray is usually taken in three views: posteroanterior (PA), oblique and lateral. At least two views about 90° apart are the minimum when a fracture is suspected, but standard imaging commonly uses three views to better assess bone alignment, joint spaces and soft tissues. Additional specialized views may be requested for specific injuries.

In anatomical position (palms facing forward), the lateral part of the hand is the thumb side — the radial border. This lateral (radial) aspect includes the thenar eminence and the first digit (thumb). The opposite, medial side is toward the little finger (ulnar border). "Lateral" denotes structures farther from the body’s midline.
