Varicella Zoster Virus IgM, in Vasai Virar

A positive varicella IgM indicates a recent or acute infection with varicella‑zoster virus, often seen in primary chickenpox and sometimes in early reactivation. IgM usually appears a few days after rash onset and can persist for weeks; false positives or negatives can occur. Results should be interpreted alongside symptoms, vaccination history, IgG serology or PCR, and clinical context to confirm diagnosis and guide management.

If testing is positive for varicella‑zoster, interpretation depends on the result: IgM suggests recent/active infection; IgG indicates past infection or immunity. Active chickenpox or shingles requires medical evaluation—antiviral treatment (e.g., acyclovir) is advised, particularly for pregnant people, newborns, or immunocompromised individuals. Isolate until lesions crust, notify healthcare providers, and consider post‑exposure prophylaxis or vaccination for susceptible contacts.

Varicella‑zoster virus can be mild in healthy children (chickenpox) but may be serious in infants, pregnant people, older adults and immunocompromised individuals. Complications include bacterial skin infections, pneumonia, encephalitis, and, when reactivated as shingles, severe nerve pain (postherpetic neuralgia). Vaccination and prompt medical care lower the risk of severe disease and complications.

Varicella‑zoster IgG is produced after exposure to the varicella‑zoster virus, most commonly following natural chickenpox infection or vaccination. It indicates past infection or immunization and usually confers long-term immunity. IgG can also be present from maternal transplacental transfer to newborns or following viral reactivation (shingles). Detection reflects prior immune response rather than active acute infection.

IgM and IgG are antibody classes with different roles and timing. IgM is a large pentamer produced first in an acute immune response, efficient at agglutination and activating complement, and indicates recent or current infection. IgG is a smaller monomer that appears later, provides long-term immunity and high-affinity pathogen neutralization, opsonization, and can cross the placenta to protect the fetus.

Varicella is often suspected clinically from fever and a pruritic vesicular rash in successive crops plus exposure history. Laboratory confirmation uses PCR of vesicle or scab material (preferred), or direct fluorescent antibody/viral culture. Serology can help: VZV IgM indicates recent infection and a rise in IgG on paired sera confirms. PCR is especially important for atypical, immunocompromised, or pregnant patients.
