Varicella Zoster Virus IgG

A positive varicella‑zoster IgG means you have antibodies from prior chickenpox infection or vaccination and are likely immune to varicella. Protection is highly likely but not absolute. It indicates past exposure rather than acute infection (IgM or rising titers diagnose recent disease). Interpretation depends on clinical context—pregnancy, immunosuppression, or occupational screening may require additional testing or advice despite a positive IgG.

Varicella zoster virus spreads mainly through inhalation of respiratory droplets and airborne particles from an infected person, and by direct contact with fluid from chickenpox or shingles blisters. It’s highly contagious, from about 1–2 days before rash onset until lesions crust. Reactivation (shingles) cannot be “caught” as shingles, but virus from shingles lesions can infect susceptible people, causing chickenpox.

Varicella‑zoster IgG is the antibody showing past infection or vaccination against chickenpox. In pregnancy, a positive IgG indicates maternal immunity and low fetal risk; a negative result means susceptibility. Non‑immune women exposed to chickenpox may be offered varicella‑zoster immune globulin or antiviral treatment. Detection of IgM suggests recent infection, which can pose fetal risks—particularly in early pregnancy—and needs urgent clinical review.

A positive varicella‑zoster IgM usually indicates a recent or acute VZV infection (chickenpox) or early reactivation (shingles). It can sometimes be false‑positive or absent in reinfection, so results must be correlated with symptoms, IgG serology, and possibly PCR. Discuss a positive IgM with your healthcare provider to confirm timing, assess severity, and determine treatment or isolation needs.

If you test positive for varicella‑zoster, isolate and avoid contact with pregnant people, newborns, and immunocompromised individuals until all lesions crust over. Contact your healthcare provider—antiviral therapy (e.g., acyclovir/valacyclovir) may be recommended, especially for older or high‑risk patients, ideally within 72 hours. Manage symptoms with pain relief, hydration and skin care. Seek urgent care for breathing problems, severe fever, eye or neurological signs.

Varicella‑zoster IgG positive means past infection or immunity — no treatment needed. If IgG negative you’re susceptible: offer varicella vaccination (two doses) and consider post‑exposure vaccination within 3–5 days.
