Valproic Acid (Sodium Valproate)

Sodium valproate and valproic acid are anticonvulsant medications used to treat various types of epilepsy (generalized and focal seizures), as mood stabilizers in bipolar disorder, and for migraine prevention. They increase brain levels of the inhibitory neurotransmitter GABA and help stabilize neuronal activity. Use requires medical supervision because of significant side effects and pregnancy-related risks.

Valproic acid is the parent acidic molecule; valproate sodium (sodium valproate) is its sodium salt used in medicines. Both deliver the valproate ion that produces anticonvulsant and mood‑stabilizing effects. The salt is more stable and water‑soluble; formulations (including divalproex) may differ slightly in absorption and dosing by molecular weight, but therapeutic effects and safety are broadly similar.

Avoid alcohol and other CNS depressants (sedatives, opioids) — they increase drowsiness and liver risk. Do not stop valproic acid abruptly. Avoid pregnancy (valproate is highly teratogenic); use effective contraception and discuss alternatives. Avoid carbapenem antibiotics and be cautious with aspirin/NSAIDs, warfarin, lamotrigine, and enzyme-inducing herbs (e.g., St John’s wort); tell your doctor about all medicines.

Valproate commonly causes nausea, vomiting, indigestion, weight gain, drowsiness, dizziness, tremor and hair loss. Less common but serious effects include liver injury, pancreatitis, blood abnormalities (low platelets), hyperammonemia with encephalopathy, and allergic reactions. In women of childbearing age it carries a high risk of major birth defects and developmental disorders and should be avoided in pregnancy unless no safer alternative exists.

Valproic acid commonly causes drowsiness, fatigue, and sedation, especially when treatment starts or doses are increased. Effects are dose-related and more likely if combined with other central nervous system depressants (e.g., benzodiazepines, opioids, alcohol). If sleepiness is severe, persistent, or interferes with daily activities, contact your prescriber — they may adjust the dose or suggest timing changes.

Bipolar disorder arises from a combination of factors: genetic predisposition, brain structure and chemical (neurotransmitter) imbalances, and alterations in neural circuits that regulate mood. Environmental triggers—such as stressful life events, substance misuse, major sleep or circadian rhythm disruption, and hormonal changes—can precipitate episodes in vulnerable people. The exact cause is multifactorial and varies between individuals.
