Urine For Hemosiderin

Hemosiderin is most commonly detected by Perls (Prussian blue) stain applied to biopsy specimens, sputum, bronchoalveolar lavage (BAL) or urinary sediment to reveal iron-containing granules in macrophages or tissues. MRI susceptibility-weighted or T2* sequences can demonstrate hemosiderin as hypointense signal in the brain or other organs. Serum iron studies are supportive but not diagnostic of tissue hemosiderin.

In hemosiderinuria, urine typically appears dark brown to cola‑colored or tea‑colored, sometimes described as rusty or blackish. This discoloration follows intravascular hemolysis when filtered hemoglobin is taken up by renal tubular cells and iron is deposited as hemosiderin; urinary hemosiderin and pigmented granules produce the characteristic dark, often persistent staining, which may last for days.

Hemosiderin appears in urine when substantial intravascular hemolysis produces hemoglobinuria and renal tubular cells take up and store iron. Causes include paroxysmal nocturnal hemoglobinuria, transfusion reactions, severe burns, malaria, mechanical hemolysis (e.g., prosthetic valves), and other severe hemolytic states. Hemosiderinuria usually develops a few days after hemoglobinuria and can persist for days to weeks; detected by Prussian blue stain.

The best treatment for hemosiderin staining is to address the underlying cause (eg, chronic venous insufficiency) with compression therapy, leg elevation, exercise and weight management. Topical lightening agents have limited benefit because pigment lies deep; persistent staining may improve with dermatologic procedures such as lasers (Q‑switched or Nd:YAG), intense pulsed light, or dermabrasion. Urine hemosiderin is an iron-containing pigment formed when hemoglobin from lysed red blood cells is broken down and stored as hemosiderin in renal tubular cells, then shed into urine. Its presence—detected by Prussian blue staining—indicates recent or ongoing intravascular hemolysis or hemoglobinuria and helps distinguish hemolysis from hematuria (intact red cells).

A urine test for hemoglobin detects blood or free hemoglobin in urine, usually with a dipstick during urinalysis. A positive result indicates hematuria or hemoglobinuria and can arise from urinary tract infections, stones, kidney disease, trauma, or systemic hemolysis. Follow-up includes urine microscopy, repeat testing, blood tests and imaging to identify the cause and guide treatment.
