A spinal ultrasound is a safe, non‑invasive imaging test that uses sound waves to visualize the spinal canal and surrounding tissues. It's most useful in infants whose spinal bones haven't fully formed, to detect conditions like spinal dysraphism, tethered cord, cysts or fluid collections, and to guide procedures (lumbar puncture/epidural). It produces no radiation and gives real‑time diagnostic and procedural images.

Ultrasound can image certain spinal structures but has limits. In infants (before posterior spinal bones fully ossify) it reliably visualizes the spinal canal and detects anomalies like tethered cord or spina bifida. In older children and adults, bone blocks sound waves, so ultrasound is limited; MRI or CT are preferred for detailed spinal imaging. Ultrasound remains useful for guided injections and assessing superficial soft tissues.

Spinal ultrasound is performed in infants, typically before 3 months and up to about 6 months of age; the optimal window is under 3 months because the posterior spinal elements are not yet ossified. It’s done promptly if skin stigmata (e.g., dimples, tufts) or neurological signs suggest spinal dysraphism. After about 6 months, MRI is preferred for evaluation.

Magnetic resonance imaging (MRI) is generally preferred for most back‑pain evaluations because it visualizes discs, spinal cord, nerve roots, bone marrow and soft tissues. Ultrasound is useful only for superficial muscles, soft‑tissue injections, or guidance of procedures and cannot reliably assess discs or the spinal canal. Imaging isn’t always required; choose MRI when symptoms are persistent, progressive, or accompanied by red flags.

MRI is generally the best scan for most spine problems because it visualizes discs, spinal cord, nerve roots and soft tissues without radiation. CT scans are better for detailed bone fractures or when MRI isn’t possible, and X-rays help assess alignment and degenerative changes. Choice depends on symptoms, suspected cause, contraindications and clinical assessment; a clinician decides the optimal imaging.

Therapeutic ultrasound can provide short-term relief for some people with back pain by reducing muscle spasm and inflammation, but high-quality evidence for meaningful or lasting benefit is limited. It’s generally not recommended as a sole long-term treatment; exercise, education, manual therapy, and targeted medications are preferred.
