Ultrasound Pelvis

Pelvic ultrasound is a painless imaging test that uses sound waves to view pelvic organs—uterus, ovaries, fallopian tubes, cervix and bladder (and prostate in men). It detects causes of pelvic pain, abnormal bleeding, masses, cysts, fibroids, ectopic pregnancy, and monitors pregnancy or infertility. It also guides procedures and assesses urinary retention or pelvic inflammatory disease. It’s noninvasive and often done transabdominally or transvaginally.

Pelvic ultrasound is generally not painful. A transabdominal scan uses gel and a full bladder, which may cause pressure or mild discomfort. A transvaginal scan can cause brief cramping or pressure but is usually well tolerated. If you experience sharp or severe pain, tell the sonographer so the exam can be paused. Follow preparation instructions to reduce discomfort.

Pelvic ultrasound can detect masses and structural changes suggesting ovarian and uterine (endometrial) cancers—often identifying ovarian tumors, cysts, or abnormal endometrial thickening. It may also reveal larger cervical or bladder masses and pelvic metastases, but is less reliable for early cervical cancer. Ultrasound suggests abnormalities but cannot confirm cancer; tissue sampling and further imaging (CT/MRI) are usually required.

Yes. Pelvic ultrasound can detect pregnancy: transvaginal ultrasound identifies an intrauterine gestational sac as early as about 4–5 weeks after the last menstrual period and can show a yolk sac and fetal pole with heartbeat by about 6–7 weeks. Transabdominal ultrasound is less sensitive and usually detects pregnancy after about 6–7 weeks. Ultrasound also helps locate pregnancies and rule out ectopic pregnancy; timing and hCG levels affect accuracy.

Pelvic ultrasound is generally very safe and uses no radiation. Risks are minimal: discomfort or pressure during transvaginal or transabdominal exams (often from a full bladder), possible light vaginal spotting, very low infection risk if the probe isn’t sterile, rare allergic reaction to the gel, and potential false-positive/negative results that may cause anxiety or prompt further testing. Discuss concerns with your clinician.

Yes. For a transabdominal pelvic ultrasound you should have a full bladder to improve imaging—typically drink about 500–1000 mL (roughly 2–4 cups) of water about an hour before the exam and avoid urinating until afterward. For a transvaginal ultrasound you’ll often be asked to empty your bladder. Always follow the specific instructions from your clinic or sonographer.
