Ultrasound Follicular Study Multiple Visit

A follicular study is usually done 2–4 times in a menstrual cycle (starting around day 8–10 and repeated every 2–3 days, sometimes daily near ovulation) to track follicle growth. There’s no strict lifetime limit—ultrasound is safe—so studies can be repeated across cycles as clinically indicated. Follow your clinician’s guidance to avoid unnecessary scans.

Multiple ovarian follicles on ultrasound can be normal (early follicular phase or after ovarian stimulation) but may also reflect polycystic ovarian morphology—classically if an ovary shows ≥12 small follicles or volume >10 mL. Ultrasound alone doesn't diagnose PCOS; clinical features (irregular periods, signs of excess androgens) and hormonal tests are needed. Clinical correlation and endocrine evaluation are recommended.

After a follicular study, next steps depend on findings: if a mature follicle is present, clinicians may give an hCG trigger and advise timed intercourse or perform intrauterine insemination (IUI), with luteal support (progesterone). If follicles are small or absent, ovulation induction (letrozole, clomiphene, or injectable gonadotropins) with repeat monitoring is used; persistent issues may prompt further tests, surgery, or IVF.

Best time to begin follicular tracking is in the early-to-mid follicular phase—typically cycle day 8–10 in a 28‑day cycle—then repeated every 2–3 days until ovulation is seen. For shorter/longer cycles or irregular periods, tracking may start earlier or be guided by a baseline scan or ovulation (LH) testing. Your fertility specialist will individualize timing based on cycle length, medications, and treatment goals.

Diagnostic obstetric ultrasound is considered safe and has no proven harmful effects. Routine care usually includes a dating/viability scan in the first trimester and an anatomy scan around 18–20 weeks; a third growth scan is done only if indicated. There’s no fixed maximum—ultrasounds may be repeated as often as clinically necessary, but unnecessary scans are discouraged.

By age 35 ovarian reserve is declining; a normal antral follicle count (AFC) is about 8–15 total across both ovaries (roughly 4–8 per ovary). Values below about 7–8 total suggest diminished reserve; very low counts (≤4–5) indicate significantly reduced reserve. AFC and AMH together give a clearer picture—discuss results with your clinician.
