Tsh (Thyroid Stimulating Hormone) 4th Generation Test

Fourth‑generation TSH refers to an ultra‑sensitive immunoassay with markedly improved analytical/functional sensitivity (around 0.01–0.02 mIU/L). It reliably detects very low TSH levels, aiding diagnosis of subclinical hyperthyroidism, monitoring thyroxine suppression in thyroid cancer, and assessing treatment response. Results still require clinical correlation and often free T4 measurement, especially when pituitary (central) thyroid disorders are suspected.

TSH of 4 mIU/L sits near the upper end of many lab reference ranges (commonly about 0.4–4.0 or 0.5–4.5 mIU/L). It can be considered borderline high; if free T4 is normal and you have no symptoms, clinicians often monitor and retest. Persistent elevation or symptoms may indicate subclinical or overt hypothyroidism and warrants medical review, pregnancy-specific consideration, and possible treatment.

TSH of about 4 mIU/L is often at the upper end of normal or mildly elevated. It can indicate subclinical hypothyroidism if free T4 is normal; symptoms, positive thyroid antibodies, pregnancy, or rising TSH increase concern. Repeat testing and check free T4 and TPO antibodies. All three matter: TSH is the most sensitive initial test and guides screening and treatment monitoring. Free T4 reflects hormone availability and helps diagnose hypo- or hyperthyroidism. Free T3 is the active hormone and is important when symptoms don’t match TSH/free T4 or in suspected T3 thyrotoxicosis. Clinicians interpret them together, not in isolation.

You can still get pregnant with a TSH of 4 mIU/L, but it’s mildly elevated and may slightly reduce fertility and raise risks of miscarriage and pregnancy complications. Repeat thyroid tests (TSH, free T4) and check thyroid antibodies. Many clinicians recommend low‑dose levothyroxine when trying to conceive or if antibodies are present to achieve pregnancy targets.
