Tissue Transglutaminase IgM Antibody

Tissue transglutaminase (tTG) antibody positive means the immune system has produced antibodies against tTG, an enzyme in the gut, typically indicating an immune reaction to gluten and suggesting celiac disease. tTG‑IgA is the usual test; confirmatory evaluation (duodenal biopsy or specialist review) and assessment for IgA deficiency or alternative IgG testing may be needed. The initial test is serology: tissue transglutaminase IgA (tTG‑IgA), with total IgA and sometimes anti‑endomysial antibody (EMA). Definitive confirmation usually requires an upper‑GI endoscopic duodenal biopsy showing villous atrophy and increased intraepithelial lymphocytes. In selected children, very high tTG‑IgA plus positive EMA can obviate biopsy. Tests must be done while the patient is eating gluten.

A tTG IgM test measures IgM-class antibodies against tissue transglutaminase, an enzyme targeted in immune responses like celiac disease. It’s less commonly used and less specific than anti‑tTG IgA/IgG. A positive result can suggest recent or atypical immune activity but requires correlation with symptoms, total IgA level, standard anti‑tTG IgA testing, and possibly intestinal biopsy for confirmation.

A high tissue transglutaminase (tTG) antibody level usually indicates an immune response to gluten, most commonly from celiac disease, and often correlates with small‑intestinal damage. After a positive celiac blood test, a gastroenterologist usually confirms diagnosis with an endoscopic duodenal biopsy; HLA testing may be used in specific cases. Patients should stay on gluten until confirmation. Evaluation includes checking for nutritional deficiencies and bone density, and referral to a dietitian. Once confirmed, a strict lifelong gluten-free diet is started with regular follow-up serology and clinical monitoring.
