Thrombin Time

Thrombin time measures how long plasma takes to form fibrin after adding thrombin. Prolongation indicates reduced or dysfunctional fibrinogen, presence of thrombin inhibitors (heparin or direct thrombin inhibitors), or excess fibrin degradation products from fibrinolysis. It helps detect fibrinogen deficiency or abnormal fibrin formation and can reveal anticoagulant contamination; a normal TT with abnormal clotting points to other pathway defects.

Thrombin time (TT) measures conversion of fibrinogen to fibrin after adding thrombin, detecting fibrinogen deficiency or inhibitors (e.g., heparin, fibrin degradation products). Activated partial thromboplastin time (aPTT) assesses intrinsic and common coagulation pathways (factors XII, XI, IX, VIII, X, V, II) and monitors unfractionated heparin; prolonged aPTT suggests factor deficiencies, inhibitors or heparin effect.

PT (prothrombin time) is a blood test measuring how long the extrinsic and common coagulation pathways take to form a clot; it's reported in seconds or as INR and is used to assess clotting function and monitor warfarin therapy. TT (thrombin time) measures conversion of fibrinogen to fibrin after added thrombin; prolongation suggests low or abnormal fibrinogen, heparin presence, or thrombin inhibitors.

PT (prothrombin time) measures the extrinsic and common coagulation pathways—sensitive to factors I, II, V, VII, X—and is reported as seconds and INR. aPTT (activated partial thromboplastin time) evaluates the intrinsic and common pathways—factors XII, XI, IX, VIII, X, V, II, I. TT (thrombin time) assesses conversion of fibrinogen to fibrin and detects low fibrinogen or inhibitors such as heparin.

TT commonly abbreviates: 1) tetanus toxoid — a vaccine to prevent tetanus; 2) thrombin time — a coagulation test measuring fibrinogen-to-fibrin conversion; 3) testosterone therapy — treatment for low testosterone; and 4) tuberculin test (Mantoux) — screening for tuberculosis infection. The exact meaning depends on clinical context; ask your clinician or check the medical record.

Liver disease commonly prolongs thrombin time (TT). Impaired hepatic synthesis lowers fibrinogen levels and can produce dysfunctional fibrinogen, both delaying thrombin‑mediated fibrin formation. Accumulation of fibrin‑split products or endogenous heparinoids in advanced liver failure also prolongs TT. Mild disease may show normal TT, but moderate to severe hepatic dysfunction typically causes a prolonged thrombin time.
