Testosterone - Total

Total testosterone is the concentration of all testosterone in the blood — free plus that bound to sex‑hormone‑binding globulin (SHBG) and albumin. It’s measured by a blood test to assess puberty, sexual function, fertility, or androgen excess. Typical adult male ranges are about 300–1000 ng/dL (10–35 nmol/L); adult female ranges are roughly 15–70 ng/dL (0.5–2.4 nmol/L), varying by lab and age.

Normal total testosterone declines with age. Typical ranges are approximately: ages 20–29: 12–35 nmol/L (350–1,000 ng/dL); 30–39: 10–30 nmol/L (300–900 ng/dL); 40–49: 9–28 nmol/L (260–800 ng/dL); 50–59: 8–25 nmol/L (230–720 ng/dL); 60+: 6–20 nmol/L (170–580 ng/dL). Low total testosterone (hypogonadism) means blood testosterone is below the normal range for age and sex. It can cause fatigue, low libido, erectile dysfunction, reduced muscle mass and bone density, mood changes, and less body hair. Causes include aging, pituitary disorders, chronic illness, obesity, medications, or genetic conditions. Diagnosis uses morning blood tests and clinical assessment; treatment varies by cause.

Total T (total testosterone) is the blood measurement of all circulating testosterone: free testosterone plus testosterone bound to sex hormone–binding globulin (SHBG) and albumin. Dangerously low testosterone in adult men typically refers to total testosterone markedly below the normal range—generally under about 200 ng/dL. Levels between roughly 200–300 ng/dL are considered low and merit evaluation; values under ~200 ng/dL are often classed as severely low and linked to significant symptoms (fatigue, low libido, muscle and bone loss). High total testosterone isn’t inherently “good.” Moderately elevated levels can boost libido, energy, muscle mass, and mood, but excessively high levels — from steroids, supplements, or medical conditions — raise risks: acne, hair loss, mood swings, sleep apnea, blood clots, reduced fertility, and prostate or cardiovascular concerns in men.
