Testosterone - Free

Free testosterone is the portion of circulating testosterone not bound to carrier proteins (sex hormone–binding globulin or albumin) and is biologically active. It can enter cells to activate androgen receptors, so it more accurately reflects hormonal availability than total testosterone alone. Free levels vary with age, health, and SHBG concentration and are used to assess androgen status and guide treatment.

Good free testosterone levels (men, measured as free T) approximate: - 20–39 years: ~9–30 pg/mL - 40–59 years: ~7–25 pg/mL - 60+ years: ~5–20 pg/mL Levels vary by lab method and individual; free T typically declines with age. Interpret results with clinical context—symptoms, total testosterone and SHBG—and discuss abnormal values with your clinician.

Free testosterone is the biologically active form, and having levels within a healthy reference range is desirable. Very low levels cause fatigue, low libido and muscle loss, while abnormally high levels—often from supplements or steroids—can cause infertility, mood changes, increased clotting, raised blood counts and cardiovascular and prostate risks. Aim for balanced, age-appropriate levels and discuss testing with your doctor.

High free testosterone can cause mood changes (irritability, aggression), acne, oily skin, increased muscle mass, body hair and male-pattern baldness. In men it may shrink testes and lower sperm production, raising infertility risk; in women it causes menstrual irregularities, hirsutism, voice deepening and reduced fertility. Long-term elevation may raise cardiovascular risk, sleep apnea and erythrocytosis; evaluate and treat causes.

Free testosterone measures the unbound, bioactive hormone and often correlates better with symptoms than total testosterone—especially when SHBG levels are abnormal (age, obesity, liver disease). However, individual assays can be unreliable, so clinicians usually assess total testosterone, SHBG (or calculate free testosterone) and clinical signs together. Free testosterone is useful but not a sole definitive indicator.

Boost free testosterone by losing excess weight, doing regular resistance and high‑intensity exercise, getting 7–9 hours’ sleep, managing stress, and limiting alcohol and smoking. Eat a balanced diet with adequate protein, healthy fats, vitamin D and zinc (supplement only if deficient). Review medications and treat underlying health issues. Consider medical evaluation before supplements or hormone therapy.
