Tacrolimus

Tacrolimus is an immunosuppressant used mainly to prevent rejection after organ transplants (commonly kidney, liver and heart) by dampening the immune response. A topical form treats inflammatory skin conditions such as moderate-to-severe atopic dermatitis (eczema) when steroids are unsuitable. It’s available as oral/IV formulations for systemic use and as an ointment for dermatologic use; dosing and monitoring are important.

Tacrolimus is high‑risk because it has a narrow therapeutic index and substantial toxicity (nephrotoxicity, neurotoxicity, hypertension, hyperglycaemia) with small concentration changes. It shows variable pharmacokinetics and many drug–drug and food interactions via CYP3A, so dosing must be individualized and requires frequent therapeutic drug monitoring. Overexposure increases infection and organ damage risk; underexposure risks rejection.

Tacrolimus is not a steroid. It’s a calcineurin‑inhibitor immunosuppressant that blocks T‑cell activation and cytokine (eg, IL‑2) production. Systemic tacrolimus prevents organ‑transplant rejection; topical formulations treat inflammatory skin conditions like atopic dermatitis. It raises infection risk and, when systemic, can cause nephrotoxicity and other adverse effects; topical use may cause local burning or irritation.

Tacrolimus binds the intracellular protein FKBP‑12, forming a complex that inhibits calcineurin phosphatase. This blocks dephosphorylation and nuclear translocation of NFAT transcription factors, reducing IL‑2 and other cytokine gene transcription. The result is suppression of T‑lymphocyte activation and proliferation, decreasing cell‑mediated immune responses used to prevent transplant rejection and treat certain autoimmune conditions.

Yes. Tacrolimus can cause both acute and chronic kidney injury by constricting renal blood vessels and promoting tubular/interstitial damage, which lowers glomerular filtration. Risk rises with high drug levels, prolonged use, drug interactions, dehydration, and preexisting kidney disease. Regular monitoring of kidney function and tacrolimus levels, dose adjustments, and avoiding other nephrotoxic drugs can help reduce the risk.

Systemic tacrolimus is usually prescribed long‑term—often lifelong—after solid‑organ transplantation to prevent rejection, with doses tailored and regular blood‑level, kidney, blood‑pressure and glucose monitoring. In other uses (e.g., topical tacrolimus for eczema) treatment may be short‑term or intermittent maintenance. Duration is individualized and should only be changed under specialist guidance. Regular follow‑up ensures dose adjustments to balance rejection risk and side effects.
