(T - Cell / B - Cell Subset) Marker : CD 5

CD5 is a surface glycoprotein expressed on T lymphocytes (and some B-cell subsets) that functions as a co-receptor modulating antigen-receptor signaling. It helps set activation thresholds, dampens T-cell receptor–mediated responses to prevent overactivation, and contributes to T-cell development and immune tolerance. Clinically, CD5 is used as a marker to identify T-cell lineage and certain lymphoproliferative disorders.

A positive CD5 marker indicates the cell expresses the CD5 surface protein, normally present on T cells and on a subset of B cells. CD5 positivity on B cells is abnormal and suggests certain B‑cell lymphoproliferative disorders—most commonly chronic lymphocytic leukemia (CLL) or mantle cell lymphoma—and prompts further diagnostic testing (immunophenotyping, genetic studies) to confirm diagnosis and guide treatment.

CD5 is expressed primarily on B-1a cells, an innate-like B cell subset found in peritoneal and pleural cavities and as a small population in peripheral blood and spleen. CD5+ B cells produce natural antibodies, can have regulatory functions, and malignant CD5+ B cells are characteristic of chronic lymphocytic leukemia. They are distinct from conventional CD5‑negative B‑2 cells.

CD5 is primarily a diagnostic immunophenotypic marker rather than a universal prognostic one. Its prognostic value depends on the cancer type: in chronic lymphocytic leukemia (CLL) and mantle‑cell lymphoma CD5 defines subtype but other factors (IGHV, TP53, ZAP‑70) better predict outcome. However, CD5‑positive diffuse large B‑cell lymphoma is associated with poorer prognosis. Thus CD5 alone is not a reliable general prognostic marker.

Major T‑cell CD markers include CD3 (pan‑T cells), CD4 (helper T cells) and CD8 (cytotoxic T cells). Other important markers are CD28 (co‑stimulation), CD25 (activation and regulatory T cells), CD45RA/CD45RO (naive versus memory), CD62L and CCR7 (homing), and CD69 (early activation). Regulatory T cells are CD4+CD25+ with low CD127 and FOXP3 expression.

CD5 and CD10 are cell-surface protein markers used in immunology and diagnostic immunophenotyping. CD5 is a glycoprotein mainly on T cells and a subset of B cells that modulates antigen-receptor signaling. CD10 (CALLA) is a cell-surface endopeptidase expressed on early B‑cell precursors and germinal-center B cells; both help classify lymphoid cells and guide diagnosis.
