A brain MRA (magnetic resonance angiography) is a noninvasive MRI test that images cerebral blood vessels to detect blockages, narrowing, aneurysms, or arterial dissection in suspected stroke. It helps determine ischemic stroke causes, assess blood flow and collateral circulation, and guide urgent treatments such as thrombolysis or thrombectomy. It may use contrast and involves no ionizing radiation; some implants can be contraindications.

An ischaemic stroke occurs when a blood clot or narrowed artery blocks blood flow to part of the brain, causing oxygen loss and rapid brain-cell death. Symptoms are sudden weakness, numbness, speech, vision or coordination problems. Major risk factors include high blood pressure, atrial fibrillation, diabetes and smoking. Immediate treatment aims to restore flow (thrombolysis or thrombectomy), followed by antiplatelet/anticoagulant therapy and rehabilitation.

No—brain MRI and MRA are different. A brain MRI produces detailed images of brain tissues (structure, tumors, stroke, inflammation), while an MRA (magnetic resonance angiography) specifically images blood vessels and blood flow in the head and neck to detect stenosis, aneurysms, or vascular malformations. MRA is a specialized MRI technique that may use flow-sensitive sequences or contrast agents.

MRI in stroke shows acute ischemia as diffusion-weighted imaging (DWI) hyperintensity with corresponding ADC signal drop (restricted diffusion). FLAIR/T2 changes appear later. MR angiography can demonstrate arterial occlusion or stenosis; perfusion imaging reveals reduced cerebral blood flow/volume and mismatch regions. Hemorrhagic stroke shows blood products with variable T1/T2 signals and susceptibility-weighted imaging (SWI) blooming, and possible contrast enhancement subacutely.

A doctor orders an MRI and MRA of the brain to image brain tissue and blood vessels: diagnose stroke, bleeding, tumors, aneurysms or vascular malformations; detect vessel narrowing or blockages; evaluate unexplained severe headaches, seizures, or new neurological deficits; plan or follow up after surgery; and assess cerebral blood flow noninvasively. MRA specifically highlights arterial and venous anatomy.

Magnetic resonance angiography (MRA) images cerebral blood vessels, so it can detect aneurysms, stenosis, occlusions or vascular malformations that cause brain injury. However, MRA does not directly show tissue damage—MRI sequences (diffusion-weighted, FLAIR) or CT are used to visualize infarcts, hemorrhage, or edema. In acute stroke, MRA can identify vessel occlusion suggesting likely brain injury requiring further imaging.
