Stress Test

A positive exercise stress test suggests reduced blood flow to the heart (ischemia) and raises concern for coronary artery disease. Your doctor will usually order further tests—stress imaging, CT or coronary angiography—and assess risk factors to confirm diagnosis and guide treatment (medications, lifestyle changes, angioplasty or bypass). False positives can occur; timely cardiology follow-up is important, especially with chest pain or high-risk features.

A stress test evaluates heart response to exercise or medication-induced stress. You’ll wear electrode patches and a blood pressure cuff while walking on a treadmill or pedaling a bike (or receive a drug if you can’t exercise). Intensity increases while staff monitor ECG, blood pressure and symptoms. Expect 10–15 minutes of exercise plus prep and recovery; report chest pain, dizziness or severe breathlessness immediately.

Stress testing assesses how the heart performs under increased workload, typically using exercise (treadmill/bike) or medications that mimic exercise. During the test, ECG, blood pressure and symptoms are monitored and sometimes combined with imaging (echo or nuclear scans). It helps detect coronary artery disease, arrhythmias, evaluate exercise capacity, and guide treatment or further testing; tests are supervised for safety.

Failing a stress test means the heart showed abnormal signs under exertion—ECG changes, symptoms, or reduced exercise capacity—suggesting inadequate blood flow or rhythm problems. It prompts more evaluation: advanced imaging, coronary angiography, or monitoring to confirm coronary artery disease or arrhythmia. Treatment may include medication changes, lifestyle modification, cardiac procedures (angioplasty/stent or bypass), or closer follow-up; urgent care is needed for ongoing chest pain.

Most people recover quickly from a cardiac stress test—within 15–30 minutes of stopping the exercise. Mild fatigue, muscle soreness or shortness of breath can persist for a few hours. If a pharmacologic agent was used, side effects may take several hours to resolve. You can usually resume normal activities the same day unless your clinician advises otherwise.

Costs vary by country and test type. In India, a basic treadmill ECG (TMT) typically costs ₹800–₹3,000; stress echocardiography ₹2,500–₹10,000; nuclear stress tests are higher. In the US, exercise ECG often costs a few hundred dollars if self-pay, while nuclear stress tests can range $1,000–$5,000+; in the UK NHS testing is usually provided free to eligible residents. Prices depend on facility and insurance.
