Sickling test

The sickling test is a simple screening blood test for sickle cell disease or trait. A blood smear or sample is exposed to a reducing agent or low-oxygen condition to induce red blood cells to deform into a sickle shape. Observation of sickled cells suggests presence of hemoglobin S; positive screens are confirmed with definitive tests like hemoglobin electrophoresis or HPLC.

A positive sickling test means red blood cells sickle when deoxygenated, indicating the presence of sickle hemoglobin. It suggests either sickle cell trait (carrier) or sickle cell disease, but isn’t diagnostic alone. Confirmatory testing (hemoglobin electrophoresis, HPLC, or genetic testing) and clinical correlation are required. Results guide counseling, family screening, and appropriate medical management to prevent or treat complications.

Another name for the sickle cell test is the sickle cell solubility test, often called the Sickledex or sickling test. This screening detects abnormal hemoglobin S; positive or unclear results are confirmed with hemoglobin electrophoresis, which precisely identifies and quantifies hemoglobin variants (such as HbS) to diagnose sickle cell trait or disease.

A normal sickling test is negative — no sickled red cells appear when the blood is deoxygenated (no visible sickling on the smear). A positive test shows sickled cells and requires confirmatory testing (hemoglobin electrophoresis or HPLC). Note: sickling screens can be unreliable in newborns or shortly after transfusion, so interpretation depends on age and clinical context.

Confirming sickle cell disease starts with newborn or screening tests (sickle solubility test or abnormal hemoglobin screen). Definitive diagnosis uses hemoglobin electrophoresis or HPLC to identify HbS, supported by peripheral blood smear (sickled cells), CBC with reticulocytosis, and DNA testing of the HBB gene for mutations or prenatal diagnosis. Genetic counseling is recommended for carriers and affected families.

A negative sickling test makes sickle cell disease unlikely but isn’t definitive. Confirmatory testing (hemoglobin electrophoresis or HPLC) is recommended, especially with symptoms, family history, recent transfusion, or low hemoglobin. Further evaluation may include full blood count, peripheral smear, and molecular tests to detect rare variants. Follow up with a hematologist or genetic counseling for clear diagnosis and guidance.
