Protein in a 24-hour urine test measures total protein excreted over a day. Small amounts are normal; elevated protein (usually >150 mg/day, or albumin >30 mg/day) suggests impaired kidney filtration from causes like glomerular disease, diabetes, hypertension, or transient factors (fever, exercise). Persistent proteinuria warrants medical evaluation, repeat testing and further kidney-focused investigations to determine cause and guide treatment.

To reduce 24-hour urine protein: control blood pressure (target ~130/80) using ACE inhibitors or ARBs when appropriate; optimize blood sugar if diabetic; lower dietary sodium and consider modest protein restriction under medical supervision; avoid NSAIDs and smoking; maintain healthy weight and exercise; treat the underlying kidney condition and have regular urine and kidney function monitoring.

Treatment depends on cause and severity. Monitor proteinuria (urine protein:creatinine ratio) and blood pressure; control hypertension with pregnancy‑safe agents (labetalol, nifedipine, methyldopa) and avoid ACE inhibitors/ARBs. Treat urinary infection if present. Severe preeclampsia needs hospitalization, magnesium sulfate for seizure prophylaxis, close maternal–fetal monitoring and often delivery. Individualized care with obstetric/nephrology input is essential.

Normally adults excrete less than 150 mg of protein in urine over 24 hours. Albumin—an important marker—should be under 30 mg/day. Values of 30–300 mg/day indicate microalbuminuria; >300 mg/day indicates overt proteinuria. Very high loss (≥3.5 g/day) suggests nephrotic‑range proteinuria and requires urgent evaluation. Testing methods include 24‑hour collection or spot albumin‑to‑creatinine ratio.

Dangerously high protein in urine usually means amounts above the macroalbuminuria/nephrotic range—over 300 mg of albumin per day (or ACR >300 mg/g) or a dipstick reading of 3+–4+. Nephrotic-range proteinuria often exceeds 3.5 g/day. These levels indicate significant kidney damage, causing edema, high blood pressure and raised cardiovascular risk, and need urgent medical evaluation.

A normal 24‑hour urine test shows total volume about 800–2,000 mL/day, protein excretion <150 mg/day, and creatinine excretion roughly 14–26 mg/kg/day. Electrolyte excretion varies: sodium typically 40–220 mmol/day and calcium under ~300 mg/day. Results depend on diet, muscle mass and hydration. Always compare with the laboratory’s reference ranges and consult your clinician for interpretation.
