Prostatic Acid Phosphatase (PAP)

Prostatic acid phosphatase (PAP) is an enzyme produced by prostate epithelial cells and secreted into semen. It hydrolyzes phosphate esters at acidic pH, helping modulate seminal fluid composition and sperm function. PAP also participates in extracellular nucleotide metabolism, influencing local signaling and immune responses. Clinically, elevated PAP levels have been associated with prostate disease and were historically used as a prostate cancer marker.

Normal prostatic acid phosphatase (PAP) values vary by laboratory and assay, but are generally considered to be about 0–3 to 0–4 ng/mL (some labs report comparable cutoffs in IU/L). Mild elevations can occur with benign prostate conditions; markedly higher levels may suggest prostate cancer or advanced disease. Use the lab’s reference range and correlate results with clinical findings and other tests.

PSA (prostate‑specific antigen) is a blood test measuring a prostate protein used to screen for or monitor prostate disease; levels can rise with cancer, benign enlargement, or inflammation. PAP (Papanicolaou) is a cervical swab cytology test (often combined with HPV testing) that examines cervical cells for precancerous or cancerous changes. PSA is for men/prostate; PAP is for cervical screening in people with a cervix.

The acid phosphatase test measures levels of the enzyme acid phosphatase in blood or body fluids to help detect and monitor conditions involving the prostate and other tissues. Elevated levels may indicate prostate disease (including prostate cancer and metastasis), bone disorders, or certain hematologic conditions. It can also be used in forensic settings to detect seminal fluid and monitor treatment response.

A PSA blood test measures the level of prostate-specific antigen, a protein produced by prostate cells. It’s used to screen for and monitor prostate conditions such as prostate cancer, benign prostatic hyperplasia, and prostatitis. Elevated PSA can result from cancer or noncancer causes, so abnormal results usually prompt repeat testing and further evaluation (imaging, biopsy) before a diagnosis.
