Platelet Count

Low platelets (thrombocytopenia) reduce blood’s ability to clot, causing easy bruising, pinpoint red spots (petechiae), prolonged bleeding from cuts, nosebleeds, bleeding gums, heavy menstrual flow, and increased risk of internal or brain hemorrhage when very low. You may feel fatigued and have prolonged bleeding after procedures. Severe cases need urgent treatment to prevent dangerous bleeding.

Normal platelet count is about 150,000–450,000/µL. Danger increases below 50,000/µL (heightened bleeding risk); below 20,000–30,000/µL spontaneous bleeding can occur; under 10,000/µL is a medical emergency with risk of life‑threatening hemorrhage. Very high counts (>450,000/µL) raise thrombosis risk and extreme levels (>1,000,000/µL) can cause clotting or paradoxical bleeding—seek prompt care, especially with bleeding or bruising.

A platelet count of 1,000/µL is critically low and life‑threatening. It greatly raises risk of spontaneous bleeding (including brain or GI), and needs urgent medical evaluation—often hospitalization, platelet transfusion, and treatment of the underlying cause (immune therapy, steroids, IVIG, or thrombopoietin agents). Avoid NSAIDs, aspirin, heavy activity, and seek immediate care if any bleeding, bruising, or severe headaches occur.

High platelet counts (thrombocytosis) can raise clot risk—deep vein thrombosis, stroke, heart attack—and may cause headaches or paradoxical bleeding. Low platelet counts (thrombocytopenia) cause easy bruising, petechiae, prolonged or heavy bleeding from cuts, gums, or nose, and, when very low, risk life‑threatening hemorrhage. Both need medical evaluation to determine cause and appropriate treatment.

To raise platelets quickly, seek prompt medical evaluation—treatment depends on the cause. Emergency options include corticosteroids, IV immunoglobulin, platelet transfusion, or thrombopoietin-receptor agonists under physician care. Stop alcohol and NSAIDs, treat infections, and correct deficiencies (iron, B12, folate) if identified. Monitor closely and go to the ER for severe bleeding, fainting, or very low counts.

Deficiencies of vitamin B12 and folate can cause low platelet counts. Both impair DNA synthesis in the bone marrow, producing megaloblastic changes and reduced platelet production (thrombocytopenia), often with anemia and fatigue. Treatment with appropriate supplementation typically restores platelet levels, but medical evaluation is needed to confirm the deficiency and exclude other causes.
