A 24-hour urine phosphorus test measures the total phosphate excreted in urine collected over a full day. It evaluates how the kidneys handle phosphate and helps diagnose causes of abnormal blood phosphate—dietary excess, parathyroid disorders, kidney or renal tubular problems—and guides management. Complete collection is essential; results are interpreted with blood phosphate, clinical history, and sometimes fractional excretion calculations.

A 24-hour urine test for kidney failure measures substances excreted over a full day—commonly creatinine clearance and total protein or albumin—to evaluate kidney filtration and detect protein loss. You collect all urine for 24 hours (discard the first morning void, save subsequent urine including the next morning), refrigerate or use preservative as instructed. Low clearance or high protein suggests reduced kidney function.

Normal urinary phosphorus (phosphate) excretion on a 24‑hour urine collection is about 0.4–1.3 grams per day (400–1,300 mg/day). Levels vary with age, dietary phosphate intake, and kidney function; spot urine tests use different reference ranges. Persistently low or high urinary phosphate may indicate metabolic, parathyroid, or kidney disorders—discuss abnormal results with your healthcare provider.

Normal 24‑hour urine results typically: volume 800–2,000 mL/day; creatinine ~14–26 mg/kg/day (≈800–2,000 mg/day depending on sex/body size); total protein <150 mg/day and albumin <30 mg/day; no detectable glucose; urinary calcium about 100–300 mg/day; electrolytes reflect intake—sodium often 40–220 mmol/day. Abnormal values require clinical evaluation.

To lower phosphorus in urine, reduce dietary phosphorus by avoiding processed foods and sodas, and limit high‑phosphorus items such as dairy, organ meats, and some nuts. Follow portion control for protein. Take prescribed phosphate binders if you have kidney disease; in advanced cases dialysis removes phosphorus. Consult your healthcare provider or a dietitian before changing medications or diet.

Unsafe serum phosphorus lies outside the normal 2.5–4.5 mg/dL (0.8–1.45 mmol/L) range. Levels above ~4.5 mg/dL signal hyperphosphatemia; values >6 mg/dL are often dangerous, causing vascular calcification and organ damage. Levels below ~2.5 mg/dL indicate hypophosphatemia; concentrations under ~1.0 mg/dL are severe and can cause muscle weakness, respiratory failure, and cellular dysfunction.
