PET Scan Brain

MRI and PET serve different purposes: MRI gives high-resolution anatomical detail and is best for structural problems (tumors, stroke, inflammation) without radiation. PET shows metabolic and functional activity, useful for dementia, epilepsy and cancer assessment, but has lower spatial resolution and uses radioactive tracers. Which is better depends on the clinical question; sometimes both are combined for complementary information.

PET (FDG‑PET) scans often miss low‑metabolic or very small cancers: indolent/low‑grade tumors (certain prostate cancers, low‑grade lymphomas), neuroendocrine/carcinoid tumors, mucinous adenocarcinomas, some renal cell carcinomas and specific lung subtypes (e.g., adenocarcinoma in situ). PET is limited for brain lesions due to high normal brain uptake, urinary‑tract tumors obscured by excreted tracer, and lesions under ~5–7 mm.

A brain PET scan is highly sensitive for metabolic and molecular changes and can detect disease earlier than structural imaging. Accuracy depends on the tracer and indication—commonly in the 80–95% range for conditions like dementia or tumor assessment. False positives and negatives can occur, so PET findings are combined with clinical evaluation and other imaging for a definitive diagnosis.

To get a brain PET scan, ask your GP or neurologist for a referral to an imaging centre or hospital; insurers often require preauthorization. Follow prep instructions—fasting, avoid caffeine and some medications, and tell staff if pregnant or diabetic. At the appointment you’ll receive a radioactive tracer, wait for uptake, then have a 20–60 minute scan. Results go to your referring doctor.

A neurologist orders a PET scan to evaluate brain function by imaging glucose metabolism and blood flow. It's used to detect and differentiate neurodegenerative diseases (like Alzheimer’s), localize seizure foci in epilepsy, characterize brain tumors and assess treatment response, and distinguish causes of cognitive decline. PET can reveal functional changes before structural scans show damage, aiding diagnosis, staging, and management plans.

MRI is generally the best scan for most brain problems because it gives high‑resolution images of soft tissue and detects tumors, inflammation, infection, demyelination, and acute ischemia (with diffusion sequences). CT is faster and preferred for suspected bleeding or skull fractures. Specialized studies—MR/CT angiography, PET, or functional MRI—assess vascular, metabolic, or functional issues.
