Myocardial Viability Assessment

Myocardial viability assessment determines whether dysfunctional heart muscle is still alive and likely to recover after restoring blood flow. It uses imaging—dobutamine stress echocardiography, perfusion SPECT/PET, and cardiac MRI with late gadolinium enhancement—to identify hibernating or stunned myocardium. Results guide decisions about revascularization versus medical therapy and help predict functional recovery and prognosis.

The most common assessment of pregnancy viability is transvaginal ultrasound to detect fetal cardiac activity and measure crown‑rump length. A fetal heartbeat visible by about 6–7 weeks generally confirms viability. If the scan is inconclusive, clinicians typically repeat ultrasound after 7–14 days or use serial quantitative beta‑hCG measurements to check for the rising levels seen in viable intrauterine pregnancies.

A cardiac viability scan evaluates whether areas of weakened heart muscle are alive and could recover with treatment. Using tests such as stress echocardiography, nuclear imaging (PET/SPECT) or cardiac MRI, it distinguishes scarred, non‑viable tissue from hibernating or stunned myocardium. Results help predict benefit from revascularisation, guide management decisions and estimate prognosis after a heart attack or chronic ischemia.

Assess myocardial viability using imaging: dobutamine stress echocardiography to detect contractile reserve; PET (FDG) or SPECT to identify perfusion–metabolism mismatch; cardiac MRI with late gadolinium enhancement to quantify scar and transmurality; and coronary angiography for anatomy. Findings guide revascularization decisions—viable myocardium (contractile reserve, perfusion–metabolism mismatch, limited scar) predicts functional recovery after revascularization.

A viability assessment evaluates whether an early pregnancy is developing normally. It includes clinical history and exam, transvaginal ultrasound to look for an intrauterine sac, embryo and fetal heartbeat, and quantitative blood tests (serial β‑hCG and sometimes progesterone) to assess hormone trends. Results classify the pregnancy as likely viable, nonviable, or inconclusive, with repeat testing or follow-up scans arranged as needed.

Cost varies by modality, location and insurance. In the US a cardiac viability study (SPECT/PET/MRI) typically ranges from about $800 to $5,000; in the UK it's usually free on the NHS but private scans may cost roughly £300–£1,200.
