Temporal bone MRI provides high-resolution soft-tissue imaging of the inner ear and adjacent structures: cochlea, vestibule, semicircular canals, internal auditory canal, and the facial and vestibulocochlear nerves. It detects tumors (vestibular schwannoma), inflammation (labyrinthitis, neuritis), CSF‑filled cysts, vascular lesions and congenital anomalies, and can identify residual/recurrent cholesteatoma with diffusion-weighted sequences. It shows limited bone detail versus CT.
