It helps diagnose causes of lower back, buttock or leg pain such as fractures, tumors, infection, inflammatory disease, nerve compression or congenital anomalies. MRI uses magnetic fields (no ionizing radiation) and may use contrast to better show abnormal tissue.

MRI is not usually necessary for uncomplicated sciatica. Diagnosis is mainly clinical, and most cases improve with conservative care. MRI is advised when red flags are present (suspected cauda equina, severe or progressive weakness, infection or cancer), symptoms persist or are disabling after about six weeks, or when surgical intervention is being considered.
