MRI Scan Shoulder Joint Contrast

Most shoulder MRIs are performed without contrast. Contrast (gadolinium intravenously or intra‑articular for an MR arthrogram) is used when greater detail is needed—e.g., suspected labral or subtle rotator cuff tears, postoperative assessment, infection, or tumor. Your referring clinician decides based on symptoms and prior imaging. Gadolinium is generally safe but avoided in pregnancy and used cautiously with severe kidney disease.

An MRI shoulder with intravenous contrast is generally not painful. You may feel a brief pinch when the IV is placed and a short warm or metallic sensation when the contrast is injected. Most people have no lasting discomfort. Rarely, allergic reactions or complications occur, especially with kidney problems. Tell the technologist about allergies, pregnancy, or kidney disease beforehand and report any unusual symptoms immediately.

For shoulder MRI, contrast is given either intravenously (gadolinium via an arm cannula during the scan) or directly into the joint for an MR arthrogram. For arthrograms, the skin is cleaned, local anesthetic applied, and a needle is placed into the glenohumeral joint under fluoroscopic or ultrasound guidance. A small volume of diluted gadolinium is injected, position confirmed, then the patient proceeds to MRI. Patients are screened and observed for reactions.

A shoulder MRI with contrast typically takes about 30–60 minutes total. The actual image acquisition is usually 20–30 minutes; adding IV placement, contrast injection and additional post‑contrast sequences or monitoring brings the appointment to roughly half an hour to an hour. Times can be longer for complex protocols, patient movement, or extra preparation.

A shoulder MRI with contrast typically costs: in the United States about $700–$3,000 (facility, region, and insurance affect price); in the United Kingdom NHS imaging is free for eligible patients, private scans usually run about £300–£800; in India roughly ₹4,000–₹12,000. Final cost depends on location, facility, whether contrast and radiologist fees are included, and your insurance coverage.

A shoulder arthrogram usually causes mild, brief discomfort: a sharp pinch when the needle goes in and pressure or aching as contrast is injected. Local anaesthetic numbs the area, and most people have only soreness or bruising for 24 to 48 hours that eases with rest and simple analgesics. Severe pain, fever, spreading redness, or increasing swelling are uncommon—report them promptly.
