MRI can detect many SI joint problems—especially inflammation (sacroiliitis), bone‑marrow edema, infections, fractures, and soft‑tissue changes. It’s more sensitive than X‑ray for early inflammatory disease but may miss purely mechanical or ligamentous dysfunction without visible structural change. CT better shows bony detail and X‑ray chronic changes. Clinical correlation and targeted injections or extra imaging may be needed.

Bilateral SI joint means both sacroiliac joints — the paired joints where the sacrum meets the pelvis — are involved. These joints transfer weight between spine and legs; when both sides are inflamed, irritated or dysfunctional, people may have lower back, buttock or groin pain, stiffness, and altered gait. Causes include injury, arthritis, pregnancy-related laxity, or degenerative changes; diagnosis and treatment include exam, imaging, pain relief, and physiotherapy.

MRI lumbar spine with SI joint screening is a noninvasive imaging test using magnetic fields to produce detailed images of the lower spine and sacroiliac joints. It evaluates discs, nerves, the spinal canal, vertebrae and SI joint inflammation, degeneration, fractures, infection or tumors. Indicated for persistent low back pain, sciatica, suspected sacroiliitis or unexplained neurologic symptoms; no ionizing radiation is used.

Bilateral sacroiliac (SI) joint injections are corticosteroid and local anesthetic injections placed into both SI joints using imaging guidance to reduce inflammation and block pain. They can provide diagnostic information by temporarily relieving pain and offer short- to medium-term symptom relief as part of a broader treatment plan. Potential risks include infection, bleeding, increased pain, allergic reaction, and rare nerve or soft-tissue injury.

SI joint pain can often be effectively managed and in many people significantly improved or resolved with conservative measures such as physical therapy, activity modification, pain relievers, injections, and lifestyle changes. Most cases don’t require surgery; surgical fusion is reserved for persistent, severe cases that fail conservative care. Early diagnosis and tailored rehabilitation increase chances of long-term relief.

An MRI of the sacroiliac (SI) joints typically takes about 20–40 minutes for the actual scan. Adding patient positioning, setup and any contrast injection can extend total appointment time to 30–60 minutes. Scan length varies with number of sequences, use of contrast, body habitus and whether additional pelvic images are required. Your imaging center will give a more exact estimate.
