Most shoulder MRIs are done without contrast, which is adequate for rotator cuff problems and general soft‑tissue evaluation. Contrast (gadolinium or an intra‑articular MR arthrogram) is used for suspected infection or tumor, complex or postoperative cases, or when detailed labral/cartilage or instability assessment is required. Discuss benefits, risks and kidney function with your doctor to decide.

After a shoulder arthrogram you should not drive if you received sedation, strong pain medicines, or still have numbness, weakness, dizziness, or significant pain. If only local anesthetic was used and sensation/muscle control has fully returned, driving is usually acceptable once you can safely operate pedals and controls. Always follow the specific advice given by the radiology team or your clinician.
