A prostate MRI can be done without contrast using biparametric MRI (T2 and diffusion-weighted imaging), which detects most clinically significant cancers. Standard multiparametric MRI usually includes gadolinium-based contrast for dynamic contrast-enhanced sequences to improve lesion characterization and help assess equivocal (PI-RADS 3) findings. Contrast is often optional and avoided with severe renal impairment or allergy; practice varies by clinical question and center.

Prostate MRI with contrast is generally not painful. You may feel brief discomfort from the IV injection (a warm flush or soreness at the needle site) and mild pressure from the scanner coil. Lying still and the scan’s noise or tight bore can cause anxiety or claustrophobia. Serious side effects are rare; allergic reactions to gadolinium are uncommon. Tell staff about kidney problems or previous reactions.

Prostate MRI uses an intravenous gadolinium-based contrast agent rather than a traditional dye. Gadolinium improves vascular and tissue contrast during dynamic contrast-enhanced (DCE) sequences in multiparametric MRI. Common agents include gadobutrol, gadoterate and gadopentetate. It’s usually safe but avoided or used cautiously in severe kidney impairment because of a rare risk of nephrogenic systemic fibrosis; discuss allergies and kidney function with your clinician.

A prostate MRI with contrast typically takes about 30 to 45 minutes for the imaging itself; including setup, positioning, IV insertion and contrast injection, total appointment time is usually 45 to 60 minutes. Some centers use extended multiparametric protocols that can lengthen the scan to about 60 minutes. Check with your imaging center for exact timing and preparation instructions.

Before a prostate MRI with contrast: arrange a recent kidney-function blood test and tell staff about allergies, kidney disease, or diabetes (you may be asked to pause metformin). Follow your centre’s fasting/hydration rules (often avoid food 4 hours, stay hydrated). Remove metal, bring a list of medicines/ID. Some centres advise a rectal enema or avoiding ejaculation 24–48 hours—follow local instructions.

For a prostate MRI you’re usually asked to change into a hospital gown and remove metal objects. You don’t typically have to fully undress, though some centers may ask you to remove underwear or lower garments if they contain metal or could cause artifacts. Follow the imaging unit’s instructions and tell staff about implants, pacemakers, or clips before scanning.
