MRI Scan Fistulogram

An MRI fistulogram is usually not very painful. The MRI scan itself is painless, but inserting a small catheter and injecting contrast can cause brief pressure, burning or discomfort. Local anesthetic or pain relief is commonly used, and staff will help minimise discomfort. Pain may be greater with active infection or tight tracts; discuss anxiolytics or analgesia with your team.

Costs vary widely by country and setting. In the United States a contrast MRI fistulogram typically runs about $1,000–4,000 out of pocket (higher at some hospitals). In the UK it’s usually covered publicly; private scans range about £300–£1,200. In India prices commonly range ₹3,000–25,000. Final cost depends on hospital, contrast use and insurance.

MRI fistulogram is a specialized MRI used to map fistulous tracts—most commonly perianal/perirectal—showing course, branching, internal openings, associated abscesses and relation to sphincter muscles. It helps classify fistula complexity, guide surgical or medical treatment planning, and monitor response after therapy. It’s non‑invasive and provides high soft‑tissue contrast for precise preoperative assessment and detection of occult extensions.

Before an MRI fistulogram, tell staff about implants, pregnancy, allergies, kidney disease and all medications (you may be asked to stop drugs like metformin). You may need a blood test for kidney function if contrast is planned. Remove jewelry/metal, wear loose clothing or a gown, and bring prior scans. For perianal studies you may be asked to empty bowels or receive a cleansing enema. Discuss claustrophobia/sedation needs.

Follow your facility’s instructions. Generally, you may need to fast 4–6 hours before an MRI fistulogram if sedation or general anesthesia is planned; clear fluids are often allowed up to 2 hours beforehand. Continue essential medications unless told otherwise. Inform staff about diabetes, allergies, pregnancy, or implanted devices, and call the imaging center if you’re unsure.

Fistula surgery is usually done under regional or general anesthesia, so you won’t feel pain during the operation. Postoperative pain is typically moderate — soreness, cramping, and discomfort are common and managed with prescribed analgesics, sitz baths, and rest. Severe or worsening pain, high fever, or heavy bleeding may signal complications and needs prompt medical review. Recovery pain usually improves over days to weeks.
